Are you a compassionate and highly motivated individual looking for an opportunity to make a real difference in cancer care? We are seeking an enthusiastic Cancer Care Coordinator to join the Thyroid Team at Leeds Cancer Centre, St Jamess Hospital.

About the Role

As a Cancer Care Coordinator, you will play a vital role in supporting patients diagnosed with thyroid cancer, working closely with the Multidisciplinary Team (MDT) alongside the Thyroid Clinical Nurse Specialist Team.

The Thyroid MDT manages the care of patients both locally and regionally, guiding them through investigations, surgery, and additional treatments such as radioactive iodine therapy. As one of the largest thyroid cancer treatment centres in the North of England, we support a diverse patient population, including both adults and paediatric patients requiring long-term follow-up care.

This role offers an exciting opportunity to contribute to the development of the team, expanding support for patients undergoing surgery in addition to oncological treatments. You will work alongside oncologists, nurse specialists, and other healthcare professionals to provide compassionate, coordinated care, ensuring the best possible experience for patients and their families.

Main duties of the job

The successful candidate will be responsible for triaging telephone calls, obtaining blood results and preparing clinics. They will also be providing supportive calls and visits to patients who are undergoing radioactive iodine treatments, providing psychological support at a role appropriate level and highlighting any concerns to the other team members.

Excellent communication and organisational skills are essential as well as the ability to work effectively as part of our team.

Following up on actions required following patient consultations in clinics, including communication to other organisations within the region, clinicians, and or supporting services

Undertake actions arising from MDT outcome discussions either under direction of the CNS or as per protocol

Take responsibility as appropriate for inputting information onto the LTHT cancer patient electronic record (PPM).

Support audit and research through measuring, collecting and collating data.

Develop and use appropriate communication skills to support regular face to face and telephone contact with patients, families, careers and external partners

Undertake Holistic Needs Assessments in line with agreed Standard Operating Procedures or Guidelines; carrying out resultant actions and or reporting to the CNS team using a patient partnership approach

Undertake phone calls to patients following discharge from hospital to ensure that the patient, family and or carer, as appropriate, is aware of future plans and appropriately refer to CNSs when there are issues that fall outside the remit of this role

Escalate concerns with regards a patients physical or psychological needs, or changes in their clinical condition to CNS colleagues within the agreed time frame

Understand the patient information pathway and support the patient, family and or carer as appropriate, to access resources where appropriate

Undertake patient pathway co-ordination responsibilities as appropriately delegated by CNSs

Coordinate and organise appointments and assessments as required within the cancer services or associated services

Assist with staff education/service improvement events

Acknowledge any limitations of competence in accepting care activities and inform a registered practitioner where appropriate

Assist in planning, implementation and evaluation of individual programmes of care for patients

Support the delivery of patient, family and carer training and education as necessary, including the use of equipment.
