Job summary

An excellent opportunity has arisen to recruit a full time Macmillan funded Cancer Care Coordinator post to work within the Neuroendocrine Tumour nursing specialty at Leeds Cancer Centre. This is a Macmillan funded post and you will benefit from being able to access their education and support programme

This is an ideal opportunity for a motivated individual to join this team to provide support for patients diagnosed with neuroendocrine cancer from diagnosis through treatment and thereafter.

The Neuroendocrine Cancer Team is based at the Leeds Cancer Centre, St Jamess Hospital. Neuroendocrine Cancers can be found in many parts of the body and they can be treated with a range of therapies including chemotherapy, radiotherapy, hormone therapies or surveillance. There would be a broad range of opportunities and experience in working with this patient group.

You will work alongside our clinical teams and will support the nurse specialists in developing, implementing, and maintaining an excellent service and improving the cancer patient experience

Main duties of the job

Your role will involve supporting patients who are attending outpatient and inpatient services. Our patients live across Yorkshire and we have a busy patient advice line and dedicated email address. You will be involved in the triaging of calls and emails for patient with queries relating to their care and treatment. You will be important in helping signpost patients to the appropriate team member or services. You will need excellent communication skills with experience of working in a health care setting.

About us

You will be a part of a small dedicated team comprising of 2 consultants and 2 nurse specialists. We work within a wider multidisciplinary team and collaborate with other services to provide care and treatment to this patient group. You will receive education support and development opportunities within the trust but also through Macmillan Cancer Charity who is funding this post.

The jobholder will be actively involved in providing direct and indirect care

for patients in support of the multidisciplinary team. The post holder will

be expected to work under direct or indirect supervision of the clinical

nurse specialists supporting the multidisciplinary team.

The jobholder will assist in the overall implementation and evaluation of

care as part of the multi professional team. The jobholder is expected to

plan, implement, deliver and maintain high standards of patient care. The

jobholder will be skilled in taking appropriate action(s) to achieve the

planned outcome.

The jobholder will contribute to the maintenance of an environment for

effective care.

The jobholder will provide a single point of contact for the patients/carers

and will be expected to work autonomously to provide this service.

The jobholder will be expected to practice in accordance within Trust

policies, guide lines defined competency boundaries and local l standard

operating procedures.

Communication and Coordination:

Communicates appropriately and sensitively in different settings, using a range of methods and styles, enhanced by underpinning knowledge and experience

Anticipates how people may feel in a given situation, responding with kindness and empathy

Clinical Responsibilities

Triaging incoming telephone calls, managing onward referrals and sign posting to other services

Following up on actions required following patient consultations in clinics, including communication to other organisations within the region, clinicians, and or supporting services

Undertake actions arising from MDT outcome discussions either under direction of the CNS/AHPs or as per protocol

Take responsibility as appropriate for inputting information onto the LTHT cancer patient electronic record (PPM).

Support audit and research through measuring, collecting and collating data.

Develop and use appropriate communication skills to support regular face to face and telephone contact with patients, families, careers and external partners

Undertake Holistic Needs Assessments in line with agreed Standard Operating Procedures or Guidelines; carrying out resultant actions and or reporting to the CNS/AHP team using a patient partnership approach

Undertake phone calls to patients following discharge from hospital to ensure that the patient, family and or carer, as appropriate, is aware of future plans and appropriately refer to CNS/AHP s when there are issues that fall outside the remit of this role

Escalate concerns with regards a patients physical or psychological needs, or changes in their clinical condition to CNS/AHP colleagues within the agreed time frame

Understand the patient information pathway and support the patient, family and or carer as appropriate, to access resources where appropriate

Undertake patient pathway co-ordination responsibilities as appropriately delegated by CNS/AHPs

Coordinate and organise appointments and assessments as required within the cancer services or associated services

Assist with staff education/service improvement events

Acknowledge any limitations of competence in accepting care activities and inform a registered practitioner where appropriate

Assist in planning, implementation and evaluation of individual programmes of care for patients

Support the delivery of patient, family and carer training and education as necessary, including the use of equipment.

Coordinate the necessary assessments, appointments or investigations to fast track people back into the system if required.

Organise and prioritise time effectively undertake duties on different wards, departments or clinical areas as and when required by service demands or patient need.

Act in such a way that safeguards the health and wellbeing of children & vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies

Assist in maintenance of accurate records and documentation, with regard to legal requirements, confidentiality and management of information.
