Speciality and Integrated Medicine (SIM) ward J14, J15 & J21.
Ward
J14 & J15 are both older adult medical ward and Ward J21 is an older adult
admissions ward. We are dedicated to providing high quality care to the older
generations of Leeds.
The
successful candidates will need to have a proven track record of providing safe
and compassionate care to their patients. Excellent inter-personal skills are
essential. A positive and flexible style and approach is necessary, and you
must be a team player.
You
will be required to work as part of a friendly and supportive team who are
creative and enthusiastic, with a track record of continually improving
performance.
You
will be actively involved in providing direct and indirect compassionate care
for patients in support of the multidisciplinary team. You will undertake a
range of delegated clinical patient care duties and can be expected to work
under direct or indirect supervision by a registered practitioner.
The jobholder will assist in the overall
implementation and evaluation of care as part of the multi professional team.
The jobholder is expected to plan, implement, deliver and maintain high
standards ofpatient
care. The jobholder will be skilled in taking appropriate action(s) and acting
on own initiative to achieve the planned outcome.
Main duties of the job
Acts
in accordance with the Nursing & Midwifery Council (NMC) Code of Conduct.
Communicate
with patients and carers to ensure they have access to appropriate information.
Maintain
and deliver compassionate patient focused care in accordance with LTHT
guidelines, policies, values and behaviours.
Provide
assessment, planning, implementation and evaluation of nursing care for
patients, ensuring documentation standards are upheld in accordance with NMC
and LTHT guidelines, whether written or computerised.
Promote
and maintain positive relationships between all staff involved in the care of
patients.
Promote
evidence based practice.
Participate
in the audit and evaluation of clinical practice in the interests of promoting
and improving quality care through action on findings.
Undertake
duties on other wards or departments as and when required by service demands or
patient need.
Act
in such a way that safeguards the health and wellbeing of children and
vulnerable adults at all times. Is familiar with and adheres to, the LTHT
safeguarding policies.
Contribute
to the promotion of a positive clinical learning environment.
Keep
up to date with developments, advances and research in individualised patient
care.
Participate
in research and or education programmes relating to professional nursing issues
and practice.
In
the absence of the Senior Charge Nurse take charge of the ward or department
and co-ordinate ward or department activities.

specialities, a broad range of jobs and a common goal of providing seamless
integrated patient centred care. We strive to be SIMply the best!

services include Older Peoples Medicine (including Same Day Emergency Care),
Diabetes, Endocrinology, HIV Outpatients, Leeds Sexual Health, Infectious
Diseases and a variety of related specialisms. We work closely with other teams
across the Trust, facilitating the admission, care and discharge of one of its
largest acute patient cohorts.
