Job summary

We are looking for a Band 3 Senior Clinical Support Worker to work 37.5 hours a week.

The NHS in Leeds works in partnership citywide to deliver diabetes care and this post will be an integral part of multi-disciplinary team working aiming to optimise the patient journey.

This is an exciting opportunity for a self-motivated and enthusiastic individual to work within the LTHT Diabetes Specialist Team based at St James Hospital.

This caring and flexible individual will provide patient centered support to the specialist diabetes outpatient clinics we provide. The role will also involve working collaboratively with the specialist nurse, dietitian, podiatry and medical team.

Experience of working in the specialty of diabetes would be an advantage but we will offer specific training relevant to the care of the patient and their families living with diabetes.

The successful candidate will need to have excellent communication skills and a passion to deliver outstanding standards of diabetes care.

The successful candidate will have:

Recent experience in working in a Secondary or Primary Care setting

NVQ 3 in Health and Social Care,

Diploma in Health Level 3, Diploma in Health and Social Care Level 3 or

equivalent health care related qualification

It may be necessary to complete additional modules within the first 12 months of employment to comply with Trust core module requirements
