The Acute Internal Medicine and Older Peoples Medicine ACP workforce at LTHT are experiencing a period of growth and due to our established educational strategy we are in a position to support the right candidates throughout the 3 year apprenticeship and beyond.

The clinical role will be undertaken across a variety of acute inpatient and ambulatory services. In addition the planned development of our SDEC services provides an exciting and rare opportunity for an AACP to utilise and develop their clinical, leadership and education knowledge and skills.

Advanced Practice is well established at Leeds Teaching Hospitals NHS Trust and is seen as integral to the delivery of high quality and safe patient care.

In addition to successful interview at LTHT, applicants will also need to secure a place on an approved university course.

Main duties of the job

This role will involve both clinical and academic training, supported by LTHT as your employer and our chosen HEE accredited Higher Education Institute (HEI). As this is an apprenticeship programme much of your learning and training will be achieved in the workplace as an integrated part of the clinical workforce.

As a TACP you will work as part of our multi-professional workforce providing direct clinical care, assessing and diagnosing patients as well as implementing and evaluating treatment and interventions alongside the medical teams. You will be supported to work towards being an autonomous practitioner across all four pillars of advanced practice and demonstrating the capabilities of the Multi-Professional Advanced Practice Framework.

Successful completion of training includes the completion of an MSc in Advanced Clinical Practice, achievement of specialty capability frameworks and demonstration of advanced level practice in leadership, research and education as well as direct clinical practice. On successful completion all trainees will be eligible to transition into a permanent Band 8a ACP post within their specialty.

Job responsibilities

The trainee advanced clinical practitioner role is typically a 3 year training programme and the post holder will be based as a paid employee, within LTHT. The apprenticeship encompasses work based and academic learning, contributing towards gaining a masters degree in advanced clinical practice at an approved Higher Education Institute (HEI), alongside successfully completing a portfolio of competency based evidence practise and consolidation of knowledge within specialist area. These competencies may be provided by your clinical service unit (CSU) or national competences depending on role.

The post holder will act as a role model to develop the knowledge to deliver advanced practice skills within their area, promoting patient centred clinical effectiveness in care, developing the skills to work autonomously in delivering independent assessment of undifferentiated and undiagnosed presentations. This will include developing the skills to preform clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients. The post holder will work closely with all members of the multidisciplinary team to develop the skills to play an integral role in the operational/strategic leadership and development of the service. At the end of the apprenticeship and successful completion of the end point assessment, you will be eligible to apply to be an advanced clinical practitioner at LTHT.

You will be required to achieve the education and practice standards as set out by the CSU at LTHT, HEIs and the Institute of Apprenticeships and undertake an end point assessment, to achieve your award in advanced clinical practice. You will be practising under direct and indirect supervision within the CSU to provide patient-centred clinical care. You will develop and build on your experience and skills to deliver specialist knowledge, underpinned by Health Education Englands four pillars of advanced clinical practice.

Undertake and complete the acquisition and development of new skills, knowledge and experience based on advanced practice competencies, which are underpinned by Health Education England 4 pillars of advanced clinical practice. The apprentice advanced clinical practitioner will complete a supported package of generic and specialist advanced practice competencies to develop the skills which may include:

Clinical -

To deliver and participate in the clinical care of patients attending the clinical area.

Provide a level of advanced and comprehensive health assessment based on a specialist body of knowledge under direct or indirect supervision.

Take responsibility for the management of patients within own caseload and for the supervision of junior members of the multidisciplinary team.

Appropriately assess, examine, investigate, diagnose and treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations within the boundaries of the role as a trainee.

Recognise patients with changing health status and to take appropriate actions

Initiate pharmacological intervention, using non-medical prescribing or patient group directives and administer medications as prescribed by others in accordance with the Leeds Teaching Hospitals policies

Communicate highly complex and highly sensitive information with patients and families and within the healthcare team.

Plan and organise complex activities for patients, including liaison with other hospital services and external agencies, and detailed planning for patient management, referral and discharge.

Be responsible for the safe use of highly complex patient monitoring and support equipment within the work area.

Undertake advanced clinical skills according to agreed protocols and service need.

Interpret laboratory and radiological results and initiate appropriate patient management

Maintain own clinical development by keeping abreast of new treatments and technologies within emergency and unscheduled care.

Improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients.

To rigorously review all aspects of the patients plan of care and identify and address issues that may result in a sub-standard service.

Contribute to the review and development of integrated care pathways and clinical guidelines to ensure a standardised approach to care.
