Job responsibilities
The
trainee advanced clinical practitioner role is typically a 3 year training
programme and the post holder will be based as a paid employee, within LTHT.
The apprenticeship encompasses work based and academic learning, contributing
towards gaining a masters degree in advanced clinical practice at an
approved Higher Education Institute (HEI), alongside successfully completing
a portfolio of competency based evidence practise and consolidation of
knowledge within specialist area. These competencies may be provided by your
clinical service unit (CSU) or national competences depending on role.
The
post holder will act as a role model to develop the knowledge to deliver
advanced practice skills within their area, promoting patient centred
clinical effectiveness in care, developing the skills to work autonomously in
delivering independent assessment of undifferentiated and undiagnosed
presentations. This will include developing the skills to preform clinical
examination, diagnosis, clinical decision making and planning of care and
treatment, diagnosis, prescribing, health promotion and safe discharge of
patients. The post holder will work closely with all members of the
multidisciplinary team to develop the skills to play an integral role in the
operational/strategic leadership and development of the service. At the end
of the apprenticeship and successful completion of the end point assessment,
you will be eligible to apply to be an advanced clinical practitioner at
LTHT.
You
will be required to achieve the education and practice standards as set out
by the CSU at LTHT, HEIs and the Institute of Apprenticeships and undertake
an end point assessment, to achieve your award in advanced clinical practice.
You will be practising under direct and indirect supervision within the CSU
to provide patient-centred clinical care. You will develop and build on your
experience and skills to deliver specialist knowledge, underpinned by Health
Education Englands four pillars of advanced clinical practice.

RESPONSIBILITY
Undertake
and complete the acquisition and development of new skills, knowledge and
experience based on advanced practice competencies, which are underpinned by
Health Education England 4 pillars of advanced clinical practice. The
apprentice advanced clinical practitioner will complete a supported package
of generic and specialist advanced practice competencies to develop the
skills which may include:
Clinical
-
To deliver and participate in the
clinical care of patients attending the clinical area.
Provide a level of advanced and
comprehensive health assessment based on a specialist body of knowledge under
direct or indirect supervision.
Take responsibility for the
management of patients within own caseload and for the supervision of junior
members of the multidisciplinary team.
Appropriately assess, examine,
investigate, diagnose and treat patients, resulting in the safe management
and appropriate referral or discharge of patients with undifferentiated and
undiagnosed presentations within the boundaries of the role as a trainee.
Recognise patients with changing
health status and to take appropriate actions
Initiate pharmacological
intervention, using non-medical prescribing or patient group directives and
administer medications as prescribed by others in accordance with the Leeds
Teaching Hospitals policies
Communicate highly complex and
highly sensitive information with patients and families and within the
healthcare team.
Plan and organise complex
activities for patients, including liaison with other hospital services and
external agencies, and detailed planning for patient management, referral and
discharge.
Be responsible for the safe use of
highly complex patient monitoring and support equipment within the work area.
Undertake advanced clinical skills
according to agreed protocols and service need.
Interpret laboratory and
radiological results and initiate appropriate patient management
Maintain own clinical development
by keeping abreast of new treatments and technologies within emergency and
unscheduled care.
Improve the quality of the patient
experience by identifying and meeting the individual clinical needs of
patients.
To rigorously review all aspects of
the patients plan of care and identify and address issues that may result in
a sub-standard service.
Contribute to the review and
development of integrated care pathways and clinical guidelines to ensure a
standardised approach to care.
Work under supervision practice
with confidence and competence within Professional Guidelines.
Ensure that accurate documentation
and records of patient care are kept.
Assist in the development of
patient management systems, processes and tools within the work area and
adjacent clinical/assessment areas.
To adapt specialist clinical
knowledge and skills to different clinical settings and influence service
delivery and patient care.
Actively implement effective
systems and processes for infection control management and relevant audit
within their clinical area.
Act in such a way that safeguards
the health and wellbeing of children and vulnerable adults at all times. Is
familiar with and adheres to, the LTHT safeguarding policies.
Raise concerns with regards to
risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing
policy and supporting processes.
To fulfil the requirements as
directed by the Nursing and Midwifery Council or Health Care Professions
Council or professional body to maintain and revalidate registration
Management
and Leadership
Choose an area of special interest
within advanced clinical practice with the intention to develop and take
forward in your ACP role.
To develop clinical leadership
skills within the Advanced Clinical Practitioner Team and LTHT
Contribute in the development of a
specialist service and act as a change agent and facilitate the change
process whilst encouraging others to be innovative and adaptable in the
approach to change.
To develop evidence-based policies
and procedures specifically required by the team.
To develop clinical competencies
for junior staff working within the team.
To develop and maintain effective
communication with the multidisciplinary team to ensure high standards of
care.
To work collaboratively with
representatives of other clinical areas, disciplines and services to provide
a seamless pathway of care.
Participate in the recruitment,
selection and retention of staff, including supervision and appraisal.
To be conversant with all Trust
policies and procedures and ensure they are correctly implemented.
To represent the Trust on National
Forums related to Advanced Clinical Practice
Education
and Training
Identify, plan and assist in the
implementation of goals for clinical development of staff working within the
service.
To develop and maintain a positive
and quality learning environment for the clinical team by providing support,
ensuring developmental requirements of junior staff are addressed.
To assist in the provision of
clinical supervision and provide the opportunity to reflect and discuss areas
of practice.
Responsible for the teaching and
delivery of core training on a range of subjects within the clinical field
for multi-professional groups.
Undertake mandatory and priority
training within the required time scales
Audit
and Research
Regularly undertake clinical audit
as part of the apprentice programme and contribute/assist in the development
of multidisciplinary research in the service area, implementing effective and
evidence-based practice.
Develop research skills and provide
support and appropriate implementation strategies for changes in clinical
practice.
Participate and contribute to any
ongoing regional and national research to evaluate the effectiveness of care
strategies related to the patient experience.
Work closely with Research and
Innovation to promote research, disseminate outcomes and findings to support
evidence based practice.
To share best practice; through
publications and attendance/presentation opportunities at conference level.
Actively promote and use the Leeds
Improvement method to improve the quality and safety of the service
Resource
Management
To apply sound financial evaluation
to proposed service developments.
Maintain accurate records in
relation to equipment used and maintained by the clinical team.
