We are excited to be advertising the post of staff nurse within our multi speciality Medically Optimised for Discharge (MOFD) service, based at the LGI on Ward L12
L12 is an exciting new ward specialising in caring for patients on the last leg of their hospital journey, supporting them to be discharged home. L12 works closely with the local and trust wide discharge teams to provide specialist care to patients who are ready to go home or continue their care and rehab journey outside of a hospital setting.
The L12 team are recognised trust wide for achieving a high standard of care for their patients and are leading the way in achieving safe and patient centred discharge planning. The team prides itself on its strong links with the wider Multi Disciplinary Team and innovations in efficient communication.
L12 is a 27 bedded ward located in the Jubilee wing at the Leeds General Infirmary at the heart of the city centre with excellent transport links and access to city centre amenities.
Main duties of the job
Successful applicants would join our Medically Optimised for Discharge ward as a registered nurse and part of a multi-disciplinary team delivering excellent care to our patients whilst developing in your role and gaining new skills. We offer a 12 month education package for all our new starters including 1:1 support on the ward from our clinical education team to help you find your feet in the first weeks and months, followed by a mix of in person practical, and remote theory teaching sessions packages to deliver neurosciences specific knowledge and skills as well as ensuring access to development opportunities in order to set you up for your nursing career.
The role is to provide and be responsible for the assessment, planning, implementation and evaluation of individualised programmes of care. Successful applicants will deliver care without supervision and take responsibility as the prime care provider for a group of patients and at all times promote confidence in our commitment to provide excellent compassionate patient care.
Successful applicants will be involved in taking charge of the ward or department as necessary in the absence of the Sister or Charge Nurse.
Successful applicants will participate in the in-service training, preceptorship and mentorship of junior staff, post basic learners and students as well as maintaining your personal development.
Successful applicants will work within an environment caring for adult patients.

The Medically Optimised for Discharge (MOFD) service is located in the Neurosciences CSU based at Leeds General Infirmary, it comprises of the Regional Centre for Neurosurgery, spinal surgery services, the Regional Centre for Stroke Thrombectomy as part of the hyper acute stroke unit, the acute stroke unit and the neurology unit. It covers inpatient and outpatient services and corresponding CNS teams.
