An exciting opportunity has arisen for an individual to apply for the new post of Family and Bereavement Support Worker at the Rob Burrow Centre for Motor Neurone Disease.

You will provide emotional, bereavement and pre-bereavement support to patients and their families affected by Motor Neurone Disease, and will work to enhance collaboration between different teams working within the area (e.g. in local hospices).

Main duties of the job

You will provide emotional, bereavement and pre-bereavement support to patients and their families affected by Motor Neurone Disease, and will work to enhance collaboration between different teams working within the area (e.g. in local hospices).

Work may take place in hospital settings or in the patients home, so the ability to travel between locations is a requirement of this role.

Providing a therapeutic and safe environment for patients and families to discuss their concerns, be it emotional, financial or spiritual is key to providing holistic patient centred care and this role is intended to support and complement the existing support structures in place.

Key relationships will be with the other social workers, family support workers, the care centre co-ordinator, medical staff, dietician, speech and language therapist and MND Association Community Support Coordinator.

This post is for the new Rob Burrow Centre for Motor Neurone Disease and is based at Seacroft Hospital (Leeds Teaching Hospitals NHS Trust). The candidate will also provide support for patients admitted to wards at Leeds General Infirmary and St James's Hospital.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

The post holder will be responsible for enhancing our service to provide a tailored whole family support service to people with Motor Neurone disease and their families. Research and audit will be key in developing and providing this service.

To undertake assessments of people and families independently to establish their emotional, social and spiritual needs. To develop and implement person specific support plans.

To support whole families with benefit applications and help them to link into services available such as through the MND Association Welfare Benefits Service and MND Association Children, Young People & Whole Families Team.

To provide bereavement support to whole families of those who have died from MND.

To perform carers assessments and help families to identify their role as a carer and support them in this.

To develop links with other relevant family support workers to enhance the citywide continuity of support for people with MND, and over time to look at ways to broaden the reach of the Family Support Worker Team across West Yorkshire

To provide education and training to the wider team regarding whole family and bereavement support.

To audit and evaluate to enable development of services and policy. To support strategic planning of MND services.
