Job summary

This is a fantastic opportunity for an experienced and enthusiastic pharmacist looking to further their career, leadership and clinical skills within the Leeds Medicines Advisory Service at the Leeds Teaching Hospitals NHS Trust (LTHT).

Our well-regarded Leeds Medicines Advisory Service incorporates the Medicines Advice Service (including our busy query answering service), medicines safety, formulary, medicines commissioning and primary care collaboration. We support and promote evidence-based decision-making about medicines, and contribute to the safe and effective use of medicines within LTHT and across the interface with primary care.

Main duties of the job

The post holder will play a key leadership role in the delivery, management and development of a highly specialised pharmacy service across the Trust, working closely at the interface between secondary and primary care. The role involves leading and directing the provision of expert prescribing advice and contributing to medicines formulary management across the primarysecondary care pathway. The post holder will ensure that medicines are used appropriately, safely and cost-effectively, in line with Trust policies, standard operating procedures, commissioned arrangements and medicines legislation.

Applicants should have experience working across the interface and have extensive experience in management and clinical pharmacy, as well as experience with Drug & Therapeutics and medicines formulary processes. You should have knowledge of prescribing practice recommendations and guidance. You will be supported by an experienced team.

The post-holder will also deliver a high quality clinical pharmacy service to allocated wards/teams and manage service provision by other pharmacy staff.

To manage and develop a highly specialised element of the pharmacy service across the Trust and at the interface between the Trust and primary care.

Provide specialist pharmaceutical professional leadership for a defined area of Pharmacy services

Provide expert opinion, both within and outside pharmacy to ensure the safe and effective delivery of medicine therapy

Be responsible for analysing and reviewing specialist information to develop policy and procedure for the defined area of responsibility

Responsible for the provision of information to Trust & Primary care committees and Clinical Service Units (CSU) regarding clinical and financial governance within their speciality, including issues around service delivery and future developments

Responsible for the co-ordination of training for pharmacists and other healthcare professionals on aspects of their specialities incorporating training on advanced skill development e.g. formulary management, risk assessment, research protocol development

Provide sessional cover in an operational service area and/or specialist clinical pharmacy provision to patients. Participates in the MMPS rota that provides services and activities across a 7-day week

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Lead and direct the provision of a highly specialised element of pharmacy services- prescribing advice and medicines formulary management across the primary/secondary care interface.

To ensure robust policies and procedures are in place to effectively manage medicines

To be responsible for development of service and policy implementation

Be responsible for individual and team performance through the setting of objectives, PDPs and appraisals. Also be responsible for recruitment strategies including undertaking skill mix reviews and succession planning.

Provide expert opinion, both within and outside pharmacy, to ensure the safe and effective delivery of medicines therapy and in relation to area of expertise.

To act as a role model and identify opportunities to provide leadership across MMPS CSU and more widely across the Trust.

Provide pharmaceutical professional leadership for a defined clinical area or service by liaison with all relevant pharmacy stakeholders and key members of any relevant CSUs.

Be responsible for ensuring that the legal and statutory requirements in relation to medicines and the specialist service elements are met and ensure that the delivery of pharmaceutical care to patients and customers in the defined areas of use meets both national and local standards

Where appropriate, lead the highly specialised pharmacy service to patients and the multidisciplinary team within a given clinical speciality.

Demonstrate the ability to provide safe, clinically effective and cost-effective use of medicines in accordance with Trust policy, standard operating procedures, commissioned treatment pathways and medicine legislation. This will include prescription review, advising on dosage and side effects and on safe administration of medicines. In specialist services this may occur for a group of defined patients as well as individuals. The post holder is expected to determine how the outcomes are best achieved in a specific circumstance.

Ensure that funding is secured for service developments through business case and negotiation with stakeholders outside pharmacy

Analyse and provide detailed information regarding medicine use and financial data for stakeholders to influence external decision making.

To provide medicine forecasting information and advice to CSUs or Trust committees on options to allow effective budget planning.

Be an expert member of Trust and primary care medicines optimisation committees relating to the use of medicines and their place in commissioned pathways with the aim of providing expert advice.

Train pharmacy staff to an appropriate level of competence depending on their role. The post holder, will participate in the training of other healthcare professionals, within the Trust and Academic partner institutions, in specialist pharmacy service related subjects.

Promote research activities of the individual and team, by identifying areas for audit and ensuring support from CSUs or Trust. The post holder will undertake and publish practice research and enable team members to do likewise.
