JOB PURPOSE/SUMMARY
Act as the Responsible Pharmacist
in line with local procedures.
Provide
advice on and ensure the safe and effective use of medicinal products to
patients at LTHT.
Provide
pharmaceutical care to patients at LTHT.

PRINCIPAL DUTIES & AREAS OF
RESPONSIBILITY
Provide pharmaceutical care to a
group of patients in various specialities on a rotational basis. Be
responsible for ensuring that the legal and statutory requirements in
relation to medicines are met and ensure that the delivery of pharmaceutical
care to patients in the defined clinical area meets both national and local
standards.
Provide professional pharmacist
cover to inpatient and outpatient dispensaries, Medicines Information,
Aseptic services and Mental Health, to meet legal requirements.
To validate prescriptions by
performing a legal and clinical professional check.
To participate in audit, the
dispensing and checking of clinical trial materials and in medicine related
intervention monitoring.
To receive prescription payments
from patients where appropriate.
To provide advice on the cost of
medicines where appropriate.
To undertake a full medication
history and review of in patients in speciality of area of practice.
To dispense and accuracy check
prescriptions and supply other pharmaceutical products.
To provide medicine related
information and advice to patients, medical and other healthcare staff. This
will include advice on medicine choice, appropriate doses, side effects, drug
interactions and effective use.
To undertake a post graduate
certificate and Diploma in Pharmacy practice.
To participate in supervision and
training of pre-registration pharmacists and other technical staff.
Be responsible for recording and
co-ordinating internal/external errors and incidents reported to them. To
learn from all medicine related events communicated through briefing.

8. INFECTION CONTROL
The
jobholder must comply at all times with the Leeds Teaching Hospitals NHS
Trust Infection Control policies, in particular by practising Universal
Infection Control Precautions. Hand hygiene must be performed before and
after contact with patients and their environment.
9. HEALTH AND SAFETY / RISK MANAGEMENT
All
staff are responsible for working with their colleagues to maintain and
improve the quality of services provided to our patients and other service
users. This includes complying at all
times with the Leeds Teaching Hospitals NHS Trust Policies, including Health
and Safety policies, in particular by following agreed security and safer
working procedures, and reporting incidents using the Trust Incident
Reporting system

11. TRAINING AND PERSONAL DEVELOPMENT
CONTINUOUS PROFESSIONAL DEVELOPMENT
The
jobholder must take responsibility in agreement with his/her line manager for
his/her own personal development by ensuring that Continuous Professional
Development remains a priority. The
jobholder will undertake all mandatory training required for the role.
12. COMMUNICATION & WORKING
RELATIONSHIPS
Internal and external
communication.
Communicates drug or medicine
related information to prescribers, nursing staff, patients and relatives.
Understanding difficulties with
relation to terminally ill and mental health patients.
Communicates within team ensuring
feedback is given. Participate in team
meetings.
Liaises with section managers on a
day-to-day basis.
Deals with external customers
giving advice to NHS Direct, other hospitals and patients giving medicine
related advice.
12. SPECIAL WORKING CONDITIONS
NB: Information in the following 4
sections should be limited to facts and must
be phrased in an appropriate manner
ie avoiding the use of emotive language,
particularly when describing the Emotional Effort required and Working Conditions.
PHYSICAL
EFFORT:
Accuracy,
dexterity, manipulation and a degree of speed in the preparation and
dispensing of medicines against written prescriptions. This involves drawing up using syringes and
weighing small amounts of raw materials.
Working
alone on wards the post holder is required to work accurately whilst under
pressure.
Post
holders may have to lift and move heavy boxes if wards request fluids at
weekends or during the evenings. These should not be more than 10kg but on
the odd occasion a trolley containing up to 15 boxes may have to be moved
from one area to another for distribution.
MENTAL
EFFORT:
Pharmacists
require long periods of concentration. When working at ward level they are
screening patients' histories and advising doctors on suitable medication
regimens. Whilst working in the dispensary they are responsible for
validating prescriptions which requires prolonged concentration and attention
to detail.
Accuracy
checking involves prolonged concentration as the post holder may be the only
person available and therefore responsible for checking out all dispensed
work
Concentration
is required when labelling and dispensing medicines against a written
prescription. Also, when preparing/checking extemporaneous products they are
expected to perform the calculations.
Controlled
Drug dispensing and checking requires frequent concentration as specific
checks must be made at all stages of labelling and dispensing. This is
managed with set breaks.
EMOTIONAL
EFFORT:
Pharmacists
work during the day on various clinical and operational rotations. They visit
wards with terminally ill patients and liaise with patients/carers on a
regular basis regarding their treatment.
