Are
you looking for an exciting and challenging role in nephrology and renal
transplantation which will improve the pharmaceutical care provided to renal
patients in both the in-patient and out-patient setting? Medicines Management
& Pharmacy Services at Leeds Teaching Hospitals are recruiting to a
vacancy within our specialist renal pharmacy team.
LTHT
Renal Service provides high quality care for the population of Leeds and the
surrounding areas as one of the largest regional specialised services for
pre-dialysis, haemodialysis and peritoneal dialysis. The Renal Department is
also the regional centre for kidney transplantation and performs around 200
transplants per year. Following repatriation of transplant immunosuppressant
medication our pharmacy team prescribe essential transplant medication and
are fully responsible for the prescription management process. We are
developing pharmacist-led medicines optimisation outpatient clinics to help
these patients get the best from their medicines.
Main duties of the job
Working
closely with the multidisciplinary team, our specialist renal pharmacists and
technicians are integral to the safe and effective use of medicines, you will
lead on significant developments and innovative strategies to improve the
medicines service available to all our patients and other users.
You
will attend consultant-led ward rounds, provide pharmacy expertise into
clinical and financial governance and strategic development of renal
services. You will facilitate the education and training of pharmacists and
other healthcare professionals in the field of renal and transplant medicines
and have opportunities to develop your clinical, leadership, managerial and
quality improvement skills.
Our
specialist renal pharmacists and technicians are integral to the safe and
effective use of medicines. You will lead on significant developments and
innovative strategies to improve the medicines service available to all our
patients and other users.

services to Renal Services.

DIMENSIONS
Leads the highly specialised
clinical pharmacy service to patients and the multidisciplinary team within a
given clinical speciality. Ensures medicines are used appropriately, safely,
and cost-effectively in accordance with Trust policy, standard operating
procedures and medicine legislation. The post holder is expected to determine
how the outcomes are best achieved in specific circumstances.
Manage other pharmacists providing
specialised clinical pharmacy service within own clinical speciality
depending on size of area.
Works directly and regularly with
consultants, senior nursing staff and managers within the clinical
speciality. Monitors and interprets
expenditure on medicines within own clinical speciality. Provides the
Directorate with regular reports to assist in the safe use of medicines, the
management of the drug budget and clinical governance issues. The report also
includes risk management such as the recording of significant clinical
interventions and near misses.
Supervise, along with other members
of their multidisciplinary team, the development and implementation of
treatment protocols, and guidelines for use of drugs within the clinical
area.
Responsible for training
pharmacists, and other health care professionals, on aspects of drug use
within the clinical area.
Provides minimum of at least one 4
hour session of service cover in the dispensary or aseptic dispensary each
week. Participates in evening, weekend and bank holiday rota and where
appropriate, out of hours services.

- Substantial post registration experience as a practising clinical pharmacist, including experience of renal medicine
- Ability to demonstrate foundation level competencies in all 6 domains of the Advanced to Consultant Level Framework (ACLF)
- Experience of dealing with highly complex clinical issues which require the pharmacist to analyse information from a range of sources
- Experience of dealing with a wide variety and grade of hospital staff (including senior medical and managerial staff) on a regular and ongoing basis
- Understanding and appreciation of local and national pharmaceutical and clinical strategies
Qualifications

- Masters qualification in Pharmacy (4 years) or equivalent qualification plus one year pre-registration experience
- Registrant of the GPhC
- Postgraduate diploma in clinical pharmacy or equivalent experience or qualification
