An
exciting opportunity has arisen for a dedicated and motivated pharmacist to
undertake a 6 month (1 day per week) fixed term cover for the Advanced
Clinical Pharmacist - Radiology at Leeds Teaching Hospitals.
The
post was created to support the continued growth in Medicines Management
needs within the Radiology Clinical Service Unit. The post holder will work
closely with colleagues within the multiple modalities of Radiology across
all sites at LTHT. Ensuring high standards of medicines governance and
safety, whilst working collaboratively on service development projects.
This
secondment would allow someone already in a substantive pharmacist post
within LTHT to provide advanced clinical pharmacy services in collaboration
with colleagues in the radiology CSU. Support of your existing line manager
would be required.
Appropriate
specialist training would be provided for the role. The current post holder
has built up strong working relationships with colleagues within both
radiology and pharmacy, all of whom provide a strong support network for this
unique role.
We
are looking for someone who is able to manage a dynamic and changing workload
with the post holder expected to utilise skills around medicines governance,
finance and clinical trials.
Main duties of the job
The
key roles for the Advanced Clinical Pharmacist include:

Pharmacist input at Radiology Quality and Governance, and Radiology Medicines
Management meetings. Provide pharmacist support with any incidents, projects,
and development of protocols and guidance.

Management of medicines shortages. Collaboratively work with pharmacy
procurement and clinical teams to agree alternatives, manage stock holdings,
provide information & updates to those required, and assess any financial
implications.

Support Drug & Therapeutics processes for individual and cohort
applications, for new or unlicensed use of medicines, and maintenance of the
LTHT medicines formulary.

Work collaboratively with the Radiology CSU and the Prescription Stationary
Group, to provide pharmacist check and authorisation of any new or renewed
Radiology charts or PGDs at LTHT.

Undertake finance reporting and stock review to support the best value for
medicines agenda.

Undertake clinical trial reviews to allow the implementation of new clinical
trials for the radiology CSU

Undertake required audits and reviews to ensure services are adhering to
local and national standards. Support Radiology to develop and implement any
action required to improve safety or patient care.

Occasionally deputise at pharmacy operational and clinical strategy
management meetings during staff shortage or planned absence.
About us
The
Radiology Advanced Clinical Pharmacist will be an active member of both
Medicines Management and Pharmacy Service and the Radiology Clinical Service
Units, working to maintain high standards of medicines use within the trust.
The Radiology Clinical Service Unit is based across all sites at LTHT,
comprised of multiple modalities including MRI, CT, Ultrasound, Radiology
Theatres and Plain Film X-Ray & Fluoroscopy. Many of these modalities
provide key radiology services within other directorates, requiring the
radiology pharmacist to work collaboratively with many clinical teams, such
as oncology, vascular, endocrine, speech and language therapy, neurology and
more. This pharmacist must be an independent, proactive and self-driven
person, whom would also thrive working with new people and staff groups to
solve problems, manage projects and implement change.

Clinical
Lead the highly specialised
clinical pharmacy service to patients and the multidisciplinary team within a
given clinical speciality.
Supports the Trust's Prescribing
Standards and challenges practice where prescribers have not adhered to the
Medicines Code.
Demonstrate the ability to provide
safe, clinically effective and cost-effective use of medicines in accordance
with Trust policy, standard operating procedures and medicine legislation.
This will include individual prescription review to optimise therapy,
advising on dosage and side effects and on safe administration of medicines.
The post holder is expected to determine how the outcomes are best achieve in
specific circumstances.
Pharmacists who are registered as
prescribers will, with the agreement of the clinical director of the
speciality, prescribe within their level of competence, to support patient
care.
Plan, manage, monitor and review
individual patient care plans.
Participate in consultant ward
rounds and multi-disciplinary team meetings.
Manage difficult and ambiguous
problems, often when information is limited.
Undertake risk management,
including investigating clinical incidents in the clinical area and ensuring
compliance with medicine legislation.
The post holder will be expected to
identify, where necessary, areas that require service changes and develop a
strategy to carry out the change through consultation with senior colleagues
within and outside the pharmacy.
Manage the introduction of new
drugs, including unlicensed drugs, into the clinical speciality.
Manage the clinical pharmacist
input into clinical trials.
Develop and update policies for the
safe, effective and economic use of medicines through discussion with medical
and nursing staff.
Communication with Primary Care
about medicines policies or guidelines specific for their area of practice.
Management
Monitor and interpret expenditure
on medicines within own clinical speciality. Provide the Directorate with
regular reports to assist in the management of the drug budget
