We have an exciting opportunity for a full time Specialist Clinical Pharmacist - split between the Anticoagulation Team and the Respiratory Team. The anticoagulant post is well integrated into the multi- disciplinary team and provides care mainly to the patients attending the outpatient clinics.
You will have the opportunity to be involved in the development of our anticoagulant service which we provide to both in and outpatients. You will be part of a team looking at piloting a new in reach service developing the specialist anticoagulation role to provide anticoagulation advice to the clinical teams in certain clinical areas.
The role will require you to work in a clinic setting based at St James Hospital where you will participate in the day-to-day delivery of the anticoagulation service including the management of warfarin.
You will be fully supported by our senior pharmacists and wider clinical team. We work closely with pharmacy technicians, doctors, and nurses. Good communication skills will be essential.It is not essential to be a prescriber for this post, but you must be willing to complete the prescribing qualification once appointed. As part your respiratory job plan you will be working on the inpatient wards and supported by our advanced clinical pharmacists and specialist clinical technician.
Main duties of the job

You will have the opportunity to be involved in the development of our anticoagulant service which we provide to both in and outpatients. You will be part of a team looking at piloting a new in reach service developing the specialist anticoagulation role to provide anticoagulation advice to the clinical teams in certain clinical areas.
The role will require you to work in a clinic setting based at St James Hospital where you will participate in the day-to-day delivery of the anticoagulation service including the management of warfarin patients using the DAWN AC dosing application.
We work closely with pharmacy technicians, doctors, and nurses. Good communication skills will be essential.
As part your respiratory job plan you will be working on the inpatient wards and supported by our advanced clinical pharmacists and specialist clinical technician. There will be an opportunity to work alongside our respiratory consultant pharmacist and be involved in upcoming projects and service development opportunities. We also have clinics within respiratory medicine and there will be opportunities to be trained to deliver these clinics.
