INFORMATION ABOUT THE DEPARTMENT

The Department of Hepatology forms part of the Leeds Liver Unit and delivers level 2 and 3 liver disease services across a large population within the North of England, including liver transplantation, acute liver failure and complex hepatology services, viral hepatitis services and a broad range of hepatology outpatient services.

The Department is affiliated to the University of Leeds with the MRC funded Centre for Bioinformatics, the Leeds Institute of Data Analysis and the NIHR funded Diagnostic Evaluation Co-operative.

This is an exciting opportunity to train in one of the leading UK hepatology centres and contribute to the Leeds Liver Units international reputation for transplantation. The successful applicant will be supervised by an academic hepatologist (Dr Ian Rowe) and a clinical hepatologist, working in collaboration with the clinical consultant team.

MEDICAL STAFF IN THE DEPARTMENT

There are 10 consultant hepatologists, one of whom is an academic hepatologist in the University of Leeds, a Specialty Doctor, 4 specialist registrars, 4 fellows (clinical/clinical-research) and 2 core medical trainees. The clinical research fellows contribute to the research programme of the unit, undertaking and developing projects and contributing to the smooth running of clinical trials. It is anticipated that approximately 50% of time will be available for research activity. The trainees participate in a resident on call rota compliant with the European Working Time Directive and also gain training and experience through outpatient clinics. The consultants operate a consultant of the week on call system and lead daily ward rounds including at weekends. There is a formal system for supervision and appraisal.

The Specialist nurse (CNS) teams include the recipient transplant co-ordinators, hepatology CNS, substance misuse CNS and 4 viral hepatitis CNSs.

KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED

Candidates should hold a primary medical qualification and be a member of one of the UK Royal Colleges of Physicians. Candidates with an equivalent qualification will be considered. Candidates must hold a full Licence to Practice with the General Medical Council. Candidates should hold a certificate in Advanced Life Support from the UK Resuscitation Council, or an equivalent and be prepared to undertake such training prior to taking up post.

DUTIES OF THE POST

(1) Description of research duties

Successful applicants will participate in the developing research programme being run by Dr Ian Rowe, academic hepatologist within the Department of Hepatology. The whole consultant team contribute to the development of the research strategy and depending on the project other clinical collaborators are likely.

The successful candidates will also contribute to the development and delivery of the clinical trials research programme within the Department of Hepatology. They will receive Good Clinical Practice and Informed Consent training, and will participate in recruiting into and reviewing patients participating in clinical trials.

(2) Description of clinical duties

Successful applicants will deliver two outpatient clinics per week in a clinic best aligned to the research project and general hepatology, viral hepatitis or liver transplantation clinics depending on training needs. Access to endoscopy training according to experience may be available, although this post primarily offers research and hepatology training. They will contribute pro rata to the Liver Units medical out of hours rota (not exceeding 1 in 7).

LEVELS OF RESPONSIBILITY

The successful candidate will contribute to the out of hours rota on the same basis as other trainees in the department, at registrar level responsibility. This is a single tier rota and MRCP or equivalent level of experience (UK completed CMT2) is essential.

The successful candidate will deliver outpatient clinics and form part of the clinical trials team, expected to comply with Good Medical Practice in research guidelines.

TEACHING AND RESEARCH/STUDY AND TRAINING

The Trust recognises the importance of research to the successful promotion and protection of health and wellbeing. Research is an integral part of the clinical research fellow/trust research doctor role and post holders will be supported in undertaking research projects and given access to appropriate research training.

You will be assigned to Dr Ian Rowes research programme and the minimum recommended 0.4 WTE will be protected time to undertake this research and training to support this work. The intention is to be able to provide 0.5 WTE time. You will also be assigned to a named clinical supervisor who will oversee your research activity and training.

This post is focussed on research training as detailed above. There will be a regular departmental research progress meeting.

There are other opportunities available, research progress and time permitting, including a weekly radiology meeting, a weekly pathology case conference and a weekly journal club with a wide variety of topics covered over the year.

A monthly governance meeting is held in the Department of Hepatology or the Liver Transplant programme, providing a well structured audit and learning environment. Participation in the hospitals mandatory audit programme and governance meetings is expected. All new starters to the Leeds Teaching Hospitals NHS Trust undertake a corporate induction, and there is a well developed programme of e-learning packages to support education in essential areas such as infection control. The Liver unit has an intranet site currently holding transplant medicine guidelines and developing as a site for other departmental guidelines. In addition the trust provides a web-based clinical guideline programme and access to Up-to-Date for all members of staff. There is a clinical sciences library based on site.

All members of the department contribute to the undergraduate medical student teaching programme.

We work with a large number of referring hospitals and offer an advisory service. Documentation and communication skills are essential, as are an ability to remain calm under pressure, work in an organised and efficient way, work with a team which does not have typical training hierarchies within it and work as a team member to ensure the efficient working of the ward. Patients access the department from great distances sometimes and the extra mile on our part to offer convenient appointments with same day investigations is appreciated. Their families are often distant and communication especially on the ward is important. The focus is on quality, timely review and intervention and understanding the patient expectation of expertise.
