Job summary

Applicants are invited for this position in paediatric and TYA Oncology and Haematology at The Leeds Childrens Hospital to commence in August 2023. This is a 10PA post

We wish to appoint an experienced doctor to this post, within a busy, dedicated paediatric and TYA oncology day-care service primarily. There are also dedicated, separate age appropriate facilities for paediatric and TYA patients and a 4 bedded bone marrow transplant unit. Experience is offered in all aspects of paediatric and TYA oncology and haematology. There is an extensive programme of multi-disciplinary team meetings, junior doctor educational meetings, clinical governance programme. The unit has a significant research portfolio and an established Candlelighters research fellowship programme.

The post would suit those with paediatric oncology or haematology experience wishing to consolidate their experience or for someone wishing to work in an out-patient based Specialty Doctor role. It is expected that the post-holder will, in conjunction with the consultants, manage patients in the clinics and in the day-care unit on weekdays, both acute patients and follow-up patients including a weekly long term follow-up clinic.

Main duties of the job

The post will be adapted, dependent on previous experience and on-going needs of the appointee. As the post-holder will be post or peri CCT, then they will be given significant clinical responsibility within the department, with consultant support.

The core details of the post include:

1. In-patient service

The post-holder will coordinate the junior doctors (training and non-training), working on the in-patient ward. It is expected that the post-holder will coordinate the ward-round, ensure that patients are reviewed according to clinical need, attend the daily consultant ward round and then coordinate and support the junior doctors and nurse practitioners in completion of routine ward jobs.

2. Out-patient service

Attend consultant led clinics - the choice of clinics will be determined by the clinical service needs, but also the interests of the appointee will be considered. The post-holder will review on-treatment and follow-up patients in clinic, assess patients for chemotherapy and arrange investigations etc. At times the post-holder will review acute walk-in patients as required. The appointee will prescribe chemotherapy (using Chemocare) in collaboration with the paediatric oncology pharmacists.

3. Bone marrow and lumbar puncture

The post-holder will perform bone marrows and lumbar punctures. In addition, once appropriately trained and assessed as competent will perform intra-thecal chemotherapy administration.

There are approximately 130 new referrals of children and teenagers with malignant disease per year. The service is delivered by a large comprehensive multi-professional team in both in-patient and out-patient including day care facilities. The centre is also a tertiary benign paediatric haematology centre with a large population of children with bleeding disorders and haemoglobinopathies. The wider childrens hospital contains the complete range of paediatric sub-specialty services including congenital cardiac surgery, renal transplantation and a supra-regional liver transplant service.

There are three in-patient wards, which are all co-located on A Floor, Clarendon Wing at Leeds General Infirmary.

1. L31 - 17 bed childrens oncology and haematology ward.

2. L32 - 4 bed paediatric and TYA stem cell transplant ward.

3. L33 - 8 bed dedicated teenage cancer trust supported, teenage oncology and haematology ward.

The out-patient/day-care ward is located close to the inpatient ward and accommodates patients requiring day-case investigations and treatment, including three general anaesthetic lists each week for bone marrow and lumbar puncture procedures.

The successful applicant will join the Paediatric Haematology and Oncology Department which is led by Dr Susan Picton (Lead Clinician).

The paediatric haematology team includes:

Dr Mike Richards (Associate Medical Director), Dr Beki James (Director of Paediatric Stem Cell Transplant) and Dr Jacob Grinfeld who provide the regional paediatric haematology service.

The paediatric oncology team includes:

Dr Susan Picton, Dr Danielle Ingham, Dr Chris Lethaby and Dr Amy Ruffle; and in the wider department will also work with Professor Adam Glaser (Clinical Lead for the NIHR Childrens Clinical Research Facility), Dr Martin Elliott, Dr Bob Phillips and Dr Simone Wilkins.

The appointee will primarily be responsible for the day to day care of the paediatric and young adult haematology inpatients undergoing bone marrow transplantation. They will be supported in this by the haematology consultants, the paediatric haematology registrar, the adult haematology registrar and the paediatric oncology team. As part of this they will:

Do a daily ward round on the stem cell transplant unit and of other paediatric haematology patients as needed

They will see patients attending acutely to the day unit

They will participate in clinics for stem cell transplant patients and for those with non malignant conditions and in a clinic in the paediatric haematology day unit

They will share responsibility for the general anaesthetic lists carrying out haematological procedures (typically lumbar punctures and bone marrow examinations)

They will participate in the multi-professional paediatric haematology and oncology clinical governance programmes

They will co-ordinate the weekly transplant nuggets teaching

If haematologically trained to FRCPath level the appointee will also share the morphology reporting with the consultants and haematology registrars.
