Job Summary

To contribute to the provision of evidence based professional midwifery services.

To provide high quality holistic perinatal midwifery care sensitive to the needs of the woman and her family. To work in partnership with the multi-disciplinary team in both the hospital and community settings, providing continuity of carer according to the local models of care. This includes partnership working with general practitioners, obstetricians, midwives, health visitors and all other agencies involved. To ensure that safe quality care is received by women and their families throughout the maternity pathway.

The post holder will complete the West Yorkshire & Harrogate Local Maternity & Neonatal System (WY&H LMNS) preceptorship package with support and as part of their professional development and transition to Band 6. During the preceptorship period the midwife will have the opportunity to provide care across the perinatal pathway in a variety of settings.

To act as a clinical role model and practice autonomously within the parameters of local and national guidance and sphere of professional practice, adhering at all times to The Code for Nurses and Midwives (NMC, 2018) and working within the policies and standards detailed by the Trust.

Main Duties and Responsibilities

Clinical

• Provide safe, high-quality evidence based care throughout the maternity pathway.
• Ensure information is communicated in a clear and unbiased way to facilitate informed decision making.

In partnership with the woman / birthing person, the midwife assesses needs, plans implements and evaluates care on an individual basis, promoting equity, equality and considering diversity and rights.

Maintain contemporaneous documentation in accordance with NMC Code.

Liaise and work collaboratively with the multi-disciplinary team and other agencies, when complex needs are identified.

• Continuously assess risks and deviations from normal and ensure timely referral, intervention and escalation.
• Consider the diverse needs of women and their families and ensure the delivery of individualised plans of care.
• Participates in provision of public health information
• Provide health/parent education either individually or in groups to maximise the benefit to families.
• Advocate for womens informed choice in the decision-making process regarding all aspects of their pregnancy birth and postnatal care.

Participates in departmental team meetings and other committees when necessary.

• Work flexibly across all maternity wards/settingsas indicated by service needs.
• May be required to participate in on call rota where appropriate
• Maintain compliance with Trust policies in relation to Infection Control to reduce HCAI
• Communicates in a clear and unbiased manner with women and families and the multi-disciplinary team
• Works within national and local guidelines in relation to midwifery practice
• Undertakes all such reasonable other duties as may be required as part of the role
• Escalates any concerns in relation to professional practice to relevant person/agency
• Liaises with outside agencies to enhance individualised care for more complex situations such as safeguarding, domestic abuse or substance misuse
• Maintains professional conduct including appearance at all times adhering to the local Trust uniform policy

Governance

• Ensure the appropriate reporting of clinical/operational incidents through the Trusts incident reporting process.
• Respond appropriately to complaints and feedback escalating as necessary.
• Actively contribute to quality improvement workstreams and Trust mandatory audits.
• Works within local and national guidance and immediately escalate rationale for any deviations from guidance.
• Actively engage in investigation of incidents or complaints as required.

Physical Resources

• Ensures that all equipment is in good working order and accesses relevant equipment training as required correctly.

Education and Training

• Ensure own clinical skills and knowledge are updated in line with current evidence-based practice and compliance with the local training needs analysis.

Complete the WY&H LMNS Preceptorship Package escalating any concerns to the lead midwife/ line manager.

Maintain a continuum of personal and professional development in accordance with the NMC recommendations that meets the requirements of NMC revalidation

Proactive approach to self-development, taking responsibility for personal and professional development and maintaining professional registration.

Participate in the Trust appraisal process.

• Support students during clinical placements and identify learning opportunities to enable the achievement of learning objectives.

Physical Effort

The post holder is required to have highly developed physical skills e.g. manual dexterity to perform abdominal palpation, vaginal examinations, taking blood samples, applying fetal scalp electrodes, performing episiotomies. Responding rapidly to emergency situations e.g. haemorrhage, neonatal resuscitation, where the ability to run to scene is required and bring appropriate equipment.

There is a daily requirement to undertake moderate physical activity for several short periods less than 20 minutes at a time e.g. transferring the mothers from bed to wheelchair/operating table to bed (using appropriate manual handling equipment). Assisting women into the lithotomy position for instrumental births and perineal suturing. Daily requirement to maintain awkward positions for less than 20 minutes at a time when supporting women giving birth in various positions including standing, squatting, all fours and in a birthing pool.

Mental Effort

Supporting women throughout the perinatal period requires psychological resilience and providing emotional and physical support

When caring for a woman/birthing person, the midwife may be required to maintain frequent concentration, particularly during client examinations in clinic or while attending to a woman/birthing person in labour, as well as during daily tasks such as checking and calculating drugs or inputting data into the computer. The duration and intensity of concentration may be unpredictable.

The post holder will need to be aware of the need to practice self-care for emotional and physical wellbeing and access support when required. The WY&H LMNS understands that the transition from student to newly qualified midwife can be challenging and has a commitment to supporting you throughout this journey.

Emotional Effort

Directly provides emotional and psychological support to women during pregnancy, labour and postpartum - daily throughout shift. Involvement in obstetric emergencies leading to exposure to highly distressing situations which may be as much as once a week. These may include shoulder dystocia, birth asphyxia, postpartum haemorrhage

Regular exposure to stressful situations varying in severity e.g. poor outcomes (unexpected transfer of babies to special care baby unit (SCBU), fetal abnormality, fetal or maternal death), supporting and caring for women in labouring having termination of pregnancy (TOP) for fetal abnormality, preparing baby for mortuary.

Working Conditions

The post holder is frequently exposed to challenging working conditions which includes exposure to bodily fluids, conflict management, shift work including nights, weekend, bank holidays and on call models.

Vaccinations The WY&H LMNS encourage and support staff to be vaccinated against flu, Hepatitis B and any other vaccinations deemed necessary.
