Job summary

The Cardio-Respiratory Clinical Service Unit are looking for an experienced Inherited Cardiac Conditions Clinical Nurse Specialist to join the team as a Sudden Cardiac Death Pathway Coordinator.

The ICC Service in Leeds is a well-established, multidisciplinary regional service that was founded in 2007 in line with National Service. It is a rapidly expanding service (receiving over 500 new patient referrals per year to adult cardiology alone) and has developed in line with the NHS England Service Specification for Inherited Cardiovascular Conditions. The service covers a population of approximately 3,000,000 and covers a wide geographical area.

LTHT are working in partnership with the The British Heart Foundation (BHF) and the Genomic Medicine Service Alliance (GMSA) to develop an integrated sudden cardiac death (SCD) pathway within Yorkshire. The post is for 18.75 hours per week and funding has been secured for 12 months.

Cardiac pathology, post-mortem genetic testing and clinical familial evaluation are the three primary components of the comprehensive personalised medical management of families who have suffered sudden unexpected deaths (SUD) due to potential genetic cardiac causes. The post's aim is to develop dedicated pathways to allow the coordinated the comprehensive assessment of family members, and to identify the cause of death and diagnose and treat relatives who are also potentially at risk of a genetic condition.

Main duties of the job

The post holder will work closely with services across the NHS and Coronial system to and Genomic Medicine Service Alliance to establish and manage a SCD sudden cardiac death (SCD) pathway within West Yorkshire and surrounding areas.

The postholder will design, establish, and promote a structured pathway for the systematic evaluation of family members following a SCD in which an inherited cardiac condition is a possible or proven diagnosis. They will be responsible for launching the new pathway across the region.

This will include:

1) working directly with pathologists to ensure that appropriate samples are retained at post-mortem

2) working directly with Coroners to ensure that families are appropriately signposted to regional ICC services

3) establishing coordinated appointments for families across adult and paediatric cardiology

4) working with cardiologists across the region to educate key stakeholders about the pathway

5) working closely with both laboratory and clinical genetics to facilitate molecular autopsy

They will be an expert practitioner in implementing the SCD pathway, leading by example and providing leadership and organisation to the multidisciplinary teams involved. They will improve the quality of the SCD pathway; working closely with professionals in the network as well as patients and their families.

This is a part time role of 18.75 hours per week.

In collaboration with the Lead Clinician for ICC, establish and lead consistent pathology referral practice for SCD including use of expert pathology and expert understanding of complex clinical information.

In collaboration with the Lead Clinician for ICC, establish and lead routine tissue retention for histopathology and DNA extraction in suitable SCD cases and lead arrangements for the retained tissue to be sent to the designated Genomic Laboratory Hub for storage where necessary.

In collaboration with the Lead Clinician for ICC, establish and lead mechanisms for standardised post-mortem genetic testing and reporting via NHS Genomic Laboratory Hubs (GLH) to support timely evaluation and cascade testing of families.

In collaboration with the Lead Clinician for ICC, lead the SCD programme with commitment to excellent quality, safety, confidentiality, accountability, reliability, communication and professional and managerial integrity.

In collaboration with the Lead Clinician for ICC, lead strategic service improvement by successfully initiating, managing and sustaining change to improve patient outcomes whilst motivating clinical staff within LTHT and the West Yorkshire region to ensure effective delivery and achievement of agreed service objectives, particularly in a changing healthcare environment.

Provide a positive role model to nursing staff. Lead and promote change in nursing and clinical practice in order to improve the recognition and management of ICC patients.

Highly developed specialist knowledge, underpinned by theory and experience

Education and Training
