Main duties of the job
We
are looking for a compassionate and skilled Registered Social Worker to join
the adult Cystic Fibrosis team at St Jamess Hospital, in Leeds. In this
role, you will work closely with medical professionals, clinical nurse
specialists, and the wider multidisciplinary team to provide a comprehensive
social work service to individuals with Cystic Fibrosis and their families
across the Yorkshire region.
You
will be responsible for managing a defined caseload, conducting assessments
of social care needs, and developing tailored care plans to support patients.
This includes providing financial support guidance, care coordination,
advocacy, crisis intervention, and emotional support.
You
will work collaboratively with patients, families, and the multidisciplinary
team to ensure integrated and holistic care. Your role will also involve
connecting patients to community resources and maintaining essential networks
to support their ongoing care.
