Job summary

A rare and exciting opportunity has arisen to become a Roald Dahl Children with Medical Complexities (CMC) Transition Nurse Specialist at Leeds Teaching Hospitals NHS Foundation Trust (LTHT) in partnership with the amazing Roald Dahl Marvellous Children's Charity.

The postholder will be a highly motivated, compassionate, and accountable individual who can work autonomously to improve the transition care for young people with medical complexity transitioning from childrens services to adult health services at LTHT. The cohort of children and young people we are looking to support with the new CNS roles are under the care of three or more medical specialties. Their medical conditions are life-long and complex, and the pressure placed on their parents/carers is relentless.

The complexities and health needs of these children and young people with CMC can result in fragmented, inconsistent, and uncoordinated care which often causes confusion and unsatisfactory outcomes for them and their families.

The Roald Dahl CMC Transition Nurse Specialist will enable proactive and inclusive services centred on the needs of the child and family. The benefits would include a reduction in emergency admissions, prevent hospitalisation, improved communication.

Main duties of the job

Main duties of the role:

To provide a high-quality CMC transition service, meeting the individual needs of children, young people, and their families.

Establish CMC transition clinics and MDT coordination of CMC across LTHT and in the community.

Providing healthcare transition support to staff and professionals at LTHT ensuring a cohesive and a co-ordinated transition process.

Coordinate care within the hospital setting and work with community teams to coordinate healthcare transition and improve access to the CMC transition service.

Provide clinical expertise on CMC transition to improve safe standards of care by active involvement in the patient journey and development of patient pathways.

Work collaboratively with all stakeholders including MDTs, CNSs, clinical teams, Consultants, AHPs, and community nursing teams across the trust and the ICB to facilitate improved patient flow, capacity, and continuity of care.

To effectively liaise with education and social care to reduce fragmentation of transition care across organisational boundaries.

Provide evidence-based healthcare transition teaching/training to CYP and their families, providing up to date evidence based verbal and written information/leaflets that is age and culturally appropriate to promote and develop their confidence to become more independent in managing their health

The post holder will be practising as an independent specialist clinical practitioner providing patient-centred and evidence-based clinical care, within an agreed scope of competence and practice. The jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.
