We are pleased to announce an exciting opportunity at Leeds Childrens Hospital. We are seeking to recruit a highly motivated and enthusiastic Childrens Nurse (RSCN/ RN Child) to lead and manage the diabetes transition service as part of the Children and Young Peoples Diabetes Nurse Specialist Team.

The Children and Young Peoples Diabetes Service in Leeds is one of the largest in the country, caring for over 550 children and young people with diabetes of different types up to their 20th birthday. It is a local, regional and supra regional centre with many families travelling a long distance to see and take advice from our very well established and highly skilled multidisciplinary team. The Childrens Diabetes Service at Leeds is at the forefront of many recent diabetes improvement initiatives with a well-respected reputation.

Main duties of the job

The post holder will be required to lead the diabetes transition team to:

Support and educate children, young people and their families to manage diabetes. This would be provided from diagnosis, including inpatient and ongoing care with outpatients/ community visits.

To manage their own caseload along with supporting other team members to manage theirs.

Continued improvement and development of services.

Coordination of services into young adult diabetes care and in ensuring the patients journey is tailored to individual needs.

Providing specialist supervision, teaching and consultation to practitioners both within and outside the organisation

Ongoing clinical audit, research and standard setting

Liaising with young adult services in primary and secondary care and Integrated Care Boards to ensure consistent care pathways and standards continue across into young adult services.

About us

The diabetes team look after approximately 60 newly diagnosed children and CYP each year. The children and YP and their families require urgent treatment, training and education and ongoing support from a highly skilled diabetes specialist nurse. CYP require 6 appointments in the 1st year and a minimum of 4 clinic appointments each year thereafter to review and support and help with the management of diabetes. In between clinics diabetes specialist nurses provide ongoing support to families, schools, colleges and other health care professionals as required to ensure the child and YP has the best support possible for their diabetes care.

Job responsibilities

The post holder will be practising as an independent specialist clinical practitioner providing patient-centred and evidence-based clinical care, within an agreed scope of competence and practice. The jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.

The jobholder will provide specialist nursing care, treatment and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources. The job holder will support the development of interdisciplinary and whole service working. As a role model they will promote the sharing of good practice and delivery of high quality, effective and efficient care. The job holder will provide training and education to staff groups, patients and families/carers.

The post holder will work in partnership with the multi-disciplinary team (MDT) to implement and improve the patient pathway and will be expected to develop new skills to further enhance the quality of the service delivered. Where appropriate, the role will include the day to day leadership and management.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Leadership

Provide leadership to the clinical team working with Matron, develop new roles/ways of working

Provide a visible clinical lead and role model to the nursing and wider team by delivering high quality compassionate care.

Support staff to provide efficient, effective, safe and quality care to patients as an expert practitioner.

Deliver a positive patient experience.

Support in the delivery of an effective, evidence based, safe, quality service within budgetary envelope.

Responsible for the day to day supervision of junior clinical staff and students within the specialist team.

Motivate staff in the delivery of quality patient care.

Has responsibility for the line management of the specialist nursing team including recruitment, performance management and workforce planning.

Deliver an effective quality service with budgetary constraints, monitors the delegated budget for the specialist nursing Teamwork with the Matron to work towards the Nursing, Midwifery and Allied Health Professionals objectives and service improvement.

Supervise clinical delivery of care competencies.

Anticipate problems and take preventative or mitigating actions, or have contingency plans available.

Investigate and respond to accidents, complaints, untoward incidents and other significant events.

Ensures that workload and performance monitoring data for the team is collected, collated and utilised to ensure the effective and efficient working of the team. Reviews this information with Matron on a regular basis.

Work with the Matron in the development of specific elements of nursing practice, reflecting the CSU and Nursing, Midwifery and Allied Health Professionals business plans.

Engage users and carers views on service delivery and development.

Propose, develop and implement policy, protocols and practice guidelines specific to the clinical specialty.

Exercises a personal duty of care in relation to equipment, resource and stock control utilising equipment and supplies appropriately.

Provide high quality care as an expert practitioner and act as a role model and resource for advice, support and guidance.

Provides specialised advice contributing to the diagnosis and care of patients.

Where appropriate to the services patient demographic, to developmentally appropriate, young person friendly healthcare to support young people; including support for transition, as they move from childrens into adult services between the ages 11-25 years.

Responsibility for ensuring setting, monitoring and seeking to continuously improve standards and the quality of patient care.

Undertake clinical procedures where dexterity and accuracy are required.

Assess, plan, implement and evaluate care and treatments using specialist clinical knowledge and clinical skills.

Make clinical assessments and treatment decisions within clearly defined policies and protocols.

Organises own clinical workload.

Undertake duties on wards or departments as and when required by service demands or patient need.

Supervise clinical staff to promote the delivery of excellent clinical standards.

Receives supervision of clinical care and outcomes from nurse specialist team leader.

Act in accordance with the relevant professional code i.e. NMC.
