The post holder will have to demonstrate they can work in a team, work flexibly, have excellent communication skills in all areas and understand differences in hospital and primary care settings.

This post has a central role working alongside Rheumatology/MSK MDTs and specialist podiatrists in this field. Knowledge and understanding of high-risk patients including Rheumatology/MSK and diabetic foot and its risk factors is essential to this post. Other areas of specialist work the LTHT podiatry team are central to are in Diabetes Limb Salvage, Haemophilia, Vascular, Renal, Nail Surgery, ward-based Podiatry care, healthcare professional training and research.

Main duties of the job

The successful candidate would have support from mentor(s) and be able to develop their skills and knowledge further by utilising the multidisciplinary teams, research teams and the specialist podiatry team within an acute hospital setting.

A desire to improve and drive to reduce waste is essential whilst working closely in MDTs and the hospital settings.

Job responsibilities

JOB PURPOSE

To work within the Podiatry team taking responsibility for high risk patients within the hospital environment. These will include consultant based referrals from hospital Multi-Disciplianary Teams (MDT) including areas of Diabetes, Rheumatology, Vascular, Renal, and Orthopaedics. Diabetes will be a key area of work. Hospital wards will also be a major area of work which include planning and delivering Training and Development across the trust. The post holder will liaise with any appropriate hospital or primary health care professional. The post holder will have the ability to make a clinical diagnosis to ensure appropriate patient care within the scope of their professional practice.

3. JOB DIMENSIONS

1. To be part of a highly motivated team, providing a high standard of clinical care to all its users with a priority for specialising in the area tissue viability of the foot.

2. To demonstrate a high level of post graduate experience with a responsibility for case loads in areas of patients with high risk of foot ulceration e.g. diabetes, rheumatology, immunossupressed, elderly, etc.

3. To act as a liaison between referring agents with regard to clinical findings, proposed treatments, outcomes and further investigations along with appropriate communication of such findings to primary care workers, multi disciplinary teams and other health care professionals. This will include referral triaging and prioritising.

4. To direct and facilitate ward foot care training. This may include:-

a) Lecture based

b) E-learning

c) Working with the Training and Development team and nurse educators

d) Delivering and evaluating training packages

5. Liaise and work with specialist nurses, consultants and Tissue Viability nurses wherever appropriate at ward level.

6. Be part of the Diabetic Limb Salvage Service (MDT) clinic and help provide wound management.

7. Take part in activities involving the Hot Foot Phone, a dedicated mobile phone line for emergency diabetic foot related calls.

8. To conduct assessments and undertake the appropriate treatment of lower limb pathology in out-patient clinics and in-patients including preoperative clinical clerking and post operative care.

9. To provide routine foot care to in and out-patients where appropriate.

10. To identify development and adhere to Leeds Teaching Hospital NHS Trust (LTHT) policies, protocols, strategies, objectives and link into business plans as part of the Podiatry Department (PD) and (LTH). Adherence to Society of Chiropodists and Podiatrists, Health and Care Professional (HCPC), Department of Health (DOH), National Institute of Health and Care Excellence (NICE) guidelines.

11. To provide mentorship to students and colleagues from Podiatry, Physiotherapy and other health care professionals.

12. To be familiar and able to deal with the administration and clerical duties of the PD.

13. To participate in the planning and delivery of appropriate in-service training to podiatry and other health professionals.

KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED

1. Degree in Podiatry.

2. Local anaesthesia certificate.

3. Registration to (HPC).

4. Ionising Radiation Medical Exposure Regulations (IRMER) certificate.

5. Maintain accurate, comprehensive and up-to-date documentation and case notes in accordance with the Legal requirements set down the HCPC, College of Podiatry, LTH Trust policies, DOH NICE guidelines and departmental protocols/policies.

6. To be able to communicate assessment and treatment results to the appropriate disciplines in the form of verbal/written reports/letters.

7. Adhere to the HCPC, College of Podiatry, DOH, NICE and Health and Safety guidelines.

8. To keep registered with the HCPC and adhere to their guidelines.

9. Clinical skills in wound management should be evidence based and updated using relevant identified means e.g. appropriate journals, courses, portfolios, professional guidelines etc.

10. To be responsible for the podiatric assessment, diagnosis, planning, implementation, delivery and evaluation of conservative and surgical podiatric interventions which may include:-

Initiating biomechanical treatment and provision of orthoses/exercises and referral to other hospital specialists or podiatrists as appropriate.

Administering local anaesthetics and other drugs subject to approved Patient Group Directives (PGD).

Using and updating PGDs, Non Medical Independent Prescribing where appropriate.

Where appropriate undertake soft tissue surgery to the foot.

Nail surgery using where appropriate incisional or excisional techniques.

Tissue closure using sutures where appropriate.

Undertake neurological and vascular assessments.

Provide foot care and footwear advice or referral to appliance department for appropriate footwear.

Independent requesting of diagnostic x-ray, ultrasound imaging or CT, MRI, Bone scans after consultation with a consultant radiologist as to the relevance.

To make clinical judgement on whether Podiatry or surgery would benefit the patient.

Independent referrals to liaison psychiatry.

11. Soft tissue surgery which may include removal of infected localised bone.

This may include:-

Obtaining a full medical and social history with particular emphasis on risk factors pertaining to surgery.

Requesting diagnostic tests such as blood tests, thrombophylic screening and liaison with Haematology departments with regards to INR values. Blood glucose tolerance tests and HbA1c.

Undertake lower limb vascular assessment using doppler, ankle brachial pressure indexes and blood pressure analysis.

Interpretation of advanced test results and there clinical significance. I.e. x-ray reports, ultrasound scan reports etc.

Requesting appropriate pre/post operative antibiotics.

12. To undertake post operative patient care which may include:-

Post op assessment/progress and monitor for signs of complications including infection, deep vein thrombosis and complex regional pain syndrome.

Wound and wound dressings management.

Suture removal where appropriate.

Advice on post operative care.

Managing discharge of the patient.

13. Be able to plan and deliver lectures to Podiatry and other Health Professionals.

14. To have the ability to be involved in audit and research.

15. Professional knowledge of wound management acquired through Podiatry degree and state registration.

16. Extensive experience in wound management services and procedures.

17. Highly experienced in assessing, diagnosis and treatment of complex high risk patients.

18. To have highly developed physical skills, precision hand eye co-ordination for prolonged periods in the use of local anaesthetics, scalpel work, surgical procedures and techniques.

19. Be able to communicate medical information or gain consent when dealing with high risk patients. These include patients with complex conditions or multiple pathologies. This includes patients with emotional, physical and psychological conditions which will require the expertise of verbal and non verbal communication tools.

20. Be responsible for maintaining personal and clinical hygiene. This includes maintenance of sterile fields where necessary.

21.To participate in the identification and trial of new equipment and products.

22.To be actively involved in the provision of foot health education and advice to patient, relatives, carers and to participate in foot health training where appropriate.
