We are delighted to offer an exciting opportunity to the right candidate to join our dynamic endoscopy service as a cancer pathway manager You will work closely alongside the clinical endoscopists and clinical nurse specialists on the lower GI nurse led pathway. Receiving referrals from primary care and recording and information on the patient journey from the point of referral to diagnostic testing and post procedure care. You will be responsible for tracking the patient throughout their journey post procedure to the point of treatment, other speciality or discharge.

Main duties of the job

-Communicate with other specialities, confirming procedures are requested and booked.

-Direct communication with patients as advised by the clinical team.

-Communicate information on the patients progress in the high-risk meetings and managers team meetings.

-Develop positive working relationships with all members of the team, liaising with medical and administrative colleagues and AQP provider services.

-Support the clinical and business teams in delivering timely care and services to meet health care needs and service delivery targets.

Main duties of the job

Work closely with other specialities, managers, administration and Independent Sectors to deliver a coordinated, consistent management of patient suspected/cancer pathways.

Monitor the patients on the data base and check daily to ensure that escalation points are adhered to.

Utilise reports in the PPM Database to monitor patients and identify delays for action.

Ensure patients are booked into next appointments in line with national standards & Trust policies, communicating with key personnel to action upgrade to fast-track where required.

Highlight to Assistant Business Managers for escalation, those patients whose progress is likely to fall outside the agreed timescales.

Liaise with other specialities and trusts to ensure that the diagnostic and treatments are sought in a timely fashion.

Ensuring the Large Polyps are tracked excel and highlighted for discussion at MDT meetings, when results etc are available.

Ensuring MDT members know which patients are listed for MDT for discussion

Ensuring that diagnostic test results, pathology and audio/visual equipment etc. are available for each patient.
