2. JOB PURPOSE
The
jobholder will be joining an established team of clinical nurse specialists
who provide care to a caseload of approximately 1250 post renal transplant
patients. The jobholder will play a key role in the initial post-transplant
phase supporting their inpatient journey. The jobholder will then go on to
support them the patients in the early transplant clinic, where the jobholder
will have the opportunity to support the patient as they navigate their new
lives post-transplant.
The
jobholder will provide specialist nursing care and advice to a specific
patient group and will contribute to the development of the speciality
service. The jobholder will ensure cost effective use of resources.
3. JOB
DIMENSIONS
The
jobholder will join the team responsible for the care of all Leeds renal
transplant patients. In the initial
phase the post holder will be instrumental in managing the inpatient
workload, including daily ward visits, delivering vital bedside ERAS training
and support, providing comprehensive discharge education, and actively
participating in ward rounds. Their focused attention during this early stage
is paramount to facilitating timely and well-informed discharges.
These
clinical visits are intended to compliment normal outpatient visits with
medical staff and to work with medical staff in the capacity of nurse support
to all recipients of renal transplants.

- Provide a clinical lead and role
model to nursing staff by delivering high quality care.
- Undertake clinical procedures where
dexterity and accuracy are required

Responsibilities
for patient and client care
- Assess, plan, implement and
evaluate care and treatments using specialist clinical knowledge and clinical
skills, advising patients and relatives as appropriate.
- Organises own clinical workload and
that of more junior members of CNS team.
- Receives supervision of clinical
care and outcomes from Clinical Nurse Specialist Team Manager.

Communication
- Maintains accurate clinical and
other records using the Leeds Teaching Hospitals Trusts documentation.

families, communicating sensitive condition related information, ensuring
they receive required information to enable them to participate in their care
delivery
- Liaise with the multi-disciplinary
team and Community Services (as appropriate) to ensure seamless provision of
care delivery
Education and Training
- Provide training and education of
learners and other junior staff
- Actively participate in the
development of own personal development plan and performance review
- Provide education and training to
patients and their carers.

and advances within the specialty
Leadership and management
- Investigate and respond to
accidents, complaints, untoward incidents and other significant events.
- Work with team managers and lead
nurse to promote the development of Clinical Nurse Specialists to undertake
the CNO Ten Key Roles as appropriate to the team.
- Under the direction of the Lead
Nurse participates in or lead the development of specific elements of nursing
practice, reflecting the CMT and Nursing Directorate business plans.
- Participates in the setting,
monitoring and improvement of
standards and quality of patient care, including those standards defined
in the National Benchmarks for
Essential Care
- Promote evidence-based practice,
wherever possible
- Engage users and carers views on
service delivery and development.
- Exercises a personal duty of care
in relation to equipment, resource and stock control utilising equipment and
supplies appropriately.
- Responsible for the day to day
supervision of junior clinical staff and students within the specialist team.
- Maintains record of workload and
performance monitoring information and reviews this on a regular basis with
team manager.
Research and Audit

All

The

There
is a frequent requirement for job holder to spend a substantial proportion of
