Abdominal Medicine and Surgery Clinical Service Unit are looking to appoint a
Renal Transplant Co-ordinator to join the existing team to provide care for
patients undergoing transplant assessment, waiting list management and
participate in an on-call rota.
The
successful candidate will work closely with other members of the MDT as well
as the wider national transplant community.
In addition, they will receive full training and support in this
exciting role.

post is advertised for 0.72 WTE, we would be open to further discussion

Main duties of the job
The
post holder will be responsible for ensuring effective interdisciplinary and
multidisciplinary partnership working with key services. They will play an
integral role in order to develop the strategic provision of this service.
They will be responsible for the development of a clinical pathway and the
overall journey of the patient ensuring that patients are managed in
accordance with national standards and guidelines.
They
will be responsible for managing their own caseload of patients and the
assessment process, referring patients appropriately to other healthcare
professionals, medical, surgical psychological and social.
They
will provide and manage an expert specialist service, developing an advanced
level of professional autonomy and accountability in the provision of care
acting as an ambassador for organ donation and transplantation
They
will participate in a 24/7, 365 days a year on call rota, receiving referrals
of organs offered for transplantation from the UK and Europe taking into
account the donor aetiology and clinical status with the consultant on call
to ensure safe, efficient and seamless co-ordination of transplant surgery.

- Minimum of 18 months experience at senior level within a specific specialist area.
- Advanced theoretical and practical knowledge of a wide range of approaches to the management of patients, demonstrated by advanced clinical reasoning.
- Evidence of reviewing current practice and research developments within a specialist area and application to clinical practice.
- Knowledge of National Health Service agenda & Specialist Nation Commissioning Group agenda.
