into a pivotal role within our dedicated Post Renal Transplant Specialist
Nurse Team for a 6 month maternity leave cover and become an integral part of
a team that makes a tangible difference.
You
will guide our patients through their crucial initial phase post-transplant.
You'll be at the forefront, providing expert care and support during their
inpatient journey. The post holder is responsible for:
Empowering patients with vital
bedside ERAS training, setting them on the path to a smoother recovery.
Facilitating seamless transitions
home through comprehensive discharge education, ensuring they feel confident
and prepared.
Collaborating closely with the
multidisciplinary team during ward rounds, contributing your specialist
knowledge to optimise patient care.
You'll
also play a key role in our busy early transplant clinics, where you'll have
the opportunity to build relationships and provide ongoing support to
patients as they navigate their new lives post-transplant.
Main duties of the job
The
job holder will be instrumental in managing the inpatient workload, including
daily ward visits, delivering vital bedside ERAS training and support,
providing comprehensive discharge education, and actively participating in
ward rounds. Their focused attention during this early stage is paramount to
facilitating timely and well-informed discharges.
Furthermore,
this role extends to the early transplant clinics, which run twice weekly and
typically involve reviewing up to 6 patients each session. Currently, the
substantive CNS in this role manages an average of 30 patient reviews per
week. The job holder will work closely with the wider transplant
multidisciplinary team including specialist pharmacists, nephrologists and
consultants. The job holder will have the opportunity to engage with
transplant education sessions and local transplant network meetings to
understand the wider transplant picture with focus on service development.
About us
The
Leeds Renal Services lies within the Abdominal Medicine Surgery (AMS)
Clinical Service unit at Leeds Teaching Hospital Trust.
The
main renal unit is at St James's University Hospital, where inpatient,
outpatient, pre-dialysis, haemodialysis, peritoneal dialysis and
transplantation services are provided. Nurse-led outpatient clinics also take
place at St James's.
The
sub-regional kidney transplant service for West, East and North Yorkshire is
