Job summary
We have an exciting opportunity for either an experienced Band 6 or a Band 5, to join and further develop their skills and knowledge with the AMS Discharge Team. This is a specialist team that provides discharge related support and education to clinical areas within the AMS Clinical Service unit, as well as managing and supporting patients and their families through complex discharge plans. All ensuring that patients discharges are safe and effective once a patient no longer requires hospital level care. This role will cover Tuesday Friday 8am 4pm AMS is a large clinical service unit focusing on but not exclusively, surgical, gastroenterology, renal and urology conditions. We comprise 12 inpatient wards, a dedicated acute assessment unit as well as multiple clinics and off site dialysis units.

Main duties of the job
The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist assessment care and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.
Current services comprise providing specialist advice support and training to ensure high quality discharge planning is undertaken by wards staff.

The discharge team works with cross CSU discharge leads within Leeds Teaching Hospitals NHS Trust, as well as community colleagues to ensure all discharges are safe and effective.
