Job summary

An exciting opportunity has arisen for the appointment of a dynamic, self-motivated Band 7 to work within the Adult Critical Care CSU to lead the Rehab team.

We are looking for an enthusiastic individual to ensure high quality patient centred care is delivered to our patient group. To explore the physical and psychological needs of patients who have required critical care. Be competent in assessment of physical and psychological needs of patients following critical illness, define rehabilitation goals and be able to refer within agreed medical and nursing protocols in order to formulate an individualized rehabilitation plan.

The successful candidate will themselves be responsible for the direct line management of a group of B6 rehabilitation coordinators and B3 Clinical Support workers who work across both main sites.

The successful candidate will be responsible for leading, coordinate and participate in the delivery and development of the rehabilitation service through the use of effective evidence based practice in line with agreed national standards for patient safety and clinical practice and Trust objectives.

Main duties of the job

As part of the role The Critical Care Rehabilitation Team Leader is responsible for the development of the Service alongside the co-ordination and supervision of the day-to-day workloads and activities of the Rehabilitation Team; consisting of three B6 and 4 B3s Senior Clinical Support Workers They will work in the ward environment and those areas caring for patients following a stay in Critical Care.

They will work independently and as part of the wider multi-disciplinary team; be accountable for the planning and delivery of all aspects of patient care in relation to the specialist area of practice and as set out in their professional code of practice.

Their responsibilities will include leading a team ensuring their developmental needs are met through appraisal and ongoing support and development. They will be expected to lead or support the Matron in practice development and change management projects. Advanced critical care and leadership skills are essential, and candidates must be able to evidence well developed communication skills. Evidence of ongoing personal development and commitment to the development of their current service will be required.

We are looking for an individual who is compassionate, energetic and has a can do attitude as well as a genuine dedication to safe and effective care for patients following a stay in Adult Critical Care.
