Job summary

An exciting opportunity has arisen to join the Critical Care Outreach Team. This role offers successful candidates the opportunity to gain experience as an Critical care Outreach Team member, contributing to the clinical assessment of the deteriorating patient. You will also participate in the education of ward based teams, to staff of all levels, pertaining to the recognition and management of the deteriorating patient

Main duties of the job

The post holder will be experienced in the field of Critical Care and will play an essential role in assisting the Deteriorating Patient Lead Nurse to provide extensive clinical advice, education and support to ward based multi-disciplinary teams across a variety of clinical areas throughout the trust.

The post holder will work as an accountable, autonomous practitioner to provide comprehensive advanced assessment and care of the deteriorating/acutely ill adult patient leading to early intervention and treatment.

The post holder will work in partnership with the Allied Healthcare Professionals Team to initiate appropriate care to prevent further physiological deterioration, identifying and referring those patients who require admission to critical care.

The post holder will act as a clinical role model, promoting quality evidence-based practice. Multi-disciplinary team liaison, education and training in the care and management of the deteriorating/acutely ill adult will be key responsibilities.

Job responsibilities

Deliver and maintain a high standard of patient focused care in accordance with Leeds Teaching Hospitals Trust guidelines and policies.

Work within frequently challenging clinical environments with high levels of physical and mental effort.

Have knowledge of pathophysiology, anatomy and physiology in order to assess, plan, implement and evaluate treatment of the critically ill patient. Able to make expert clinical judgements involving complex situations which requires analysis and interpretation to decide on the best course of action/treatment.

Competently and confidently care for the deteriorating/acutely ill adult utilising expert critical care skills and knowledge in a variety of clinical settings with in direct supervision as part of the multi-disciplinary team

Provide rapid response and support for patients identified by the National Early Warning Score.

Undertake a comprehensive advanced patient assessment based on history taking and clinical examination and implement a management/treatment plan.

Provide direct patient care and support at the bedside until the patient is stabilised or transferred to an area with a higher level of care. Initiate therapeutic interventions for example to manage haemodynamic and respiratory instability.

Refer to other members of the MDT and other specialities as appropriate.

Participate in the stabilisation and transfer of critically ill patients to an appropriate environment.

Competent in advanced clinical skills such as venepuncture, cannulation, arterial stabs, fine bore nasogastric tube insertion, tracheostomy decannulation, airway management and cardiopulmonary resuscitation.

Prescribe and administer a range of drugs by patient group directives.

Accurately request pathology and radiological investigations and make clinical decisions based upon the results.

Utilise highly developed communication and negotiation skills to deal with challenging/conflict situations ensuring patient centred care.

Work autonomously taking responsibility for own decision making, referring to the appropriate physician/ICU Consultant/Outreach Team Leader when advice and further clinical input is needed or when care and treatment goes beyond normal service parameters or scope of professional practice.

Carry the crash bleep and respond to 2222 calls.

Carry the critical care bed managers bleep out of hours, liaising with the sisters/charge nurses to co-ordinate flow throughout critical care.

Respect patients rights to participate in decisions about their care and be able to advise them on complex and personal matters.

Communicate highly sensitive information to patients and their relatives, showing empathy and providing psychological support.

Maintain up to date clinical records in accordance with the most up-to-date NMC and Trust guidance.

Act as a clinical role model, which reflects the philosophy, beliefs and clinical standards of the outreach service.

Maintain a safe working environment.

Demonstrate an understanding of key infection prevention and control policies including, hand hygiene, MRSA, transfer of patients.

Demonstrate individual responsibility for infection prevention and control, challenging non-compliance with policy.
