Job Summary PMRT Coordination & Delivery
- Lead, manage, and coordinate the full PMRT process for all applicable perinatal deaths.
- Ensure reviews are completed within national timescales and to required standards.
- Facilitate multidisciplinary review meetings, ensuring appropriate attendance and highquality discussion.
- Maintain accurate, timely, and confidential records of all reviews.
- Maintain an accurate and accessible PMRT database; generate reports for local, regional, and national use.
- Provide expert advice on PMRT methodology, standards, and best practice.
- Ensure compliance with Maternity Incentive Scheme and Saving Babies Lives programmes
Other Mortality and Morbidity
- Lead, manage, and coordinate the full Mortality and Morbidity process for all applicable gynaecology deaths.
Parental and family Engagement
- Ensure parents are offered the opportunity to contribute to the review and that their perspectives are incorporated sensitively and effectively.
- Ensure all families receive compassionate communication and are fully informed of the review process.
- Work closely with Bereavement Midwives and Patient Experience teams to ensure family feedback drives service improvement.
- Represent the parent voice within the PMRT process.
Why you should join our Team We are seeking a motivated and experienced Band 7 PMRT Lead Midwife to lead the implementation, quality assurance and ongoing development of the Perinatal Mortality Review Tool (PMRT) within our maternity services.
The postholder will provide expert leadership for perinatal mortality reviews, ensuring timely, highquality multidisciplinary reviews in line with national guidance. You will work collaboratively with clinical teams, governance leads and external partners to drive learning, improve safety and support excellence in maternity care and engage with compassion with families and service users.
You will be a registered midwife with significant clinical experience, strong leadership and communication skills, and a proven interest in governance, audit and quality improvement. Experience of PMRT, perinatal bereavement care and multidisciplinary working is desirable.

What you will bring to the role The Womens CSU Quality and Safety team leader – PMRT Lead will work under the direction and guidance of the Womens CSU Governance and Quality Lead. They will be responsible for the line management of the band 6 Nurses/Midwives and the administrative staff based in the Quality and Safety team.
The key purpose of the role is to support the Womens CSU Governance and Quality lead to deliver the patient safety and quality improvement agenda. They will contribute to the development of governance and quality improvement processes to ensure the provision of high quality care standards, prevention of avoidable harm and avoidable death by using evidence-based best practice, lessons learned and data to identify incident themes, and areas for improvement.
The post holder will monitor and review incidents and investigations and escalate any areas of concern to the Womens CSU Governance and Quality Lead. They will identify opportunities for shared learning of highly complex, highly sensitive or contentious information to members of staff and contribute to the dissemination of information and learning to the wider multidisciplinary team. This may include the presentation of information which requires the use of a range of developed interpersonal and communication skills when communicating
The post holder will be responsible for daily oversight and monitoring of all CSU internal and external reporting processes linked to adverse incidents, morbidity and mortality.
