Applications are invited for a Clinical Fellowship in the intestinal failure/nutrition team within the Department of Gastroenterology, at St Jamess University Hospital in Leeds.
This is an exciting opportunity to work as a key member of one of the UKs busiest home parenteral nutrition teams. We look after approximately 180 home parenteral nutrition patients and this post supports both complex home patients and also 30-40 inpatients who receive intravenous nutritional support on a daily basis.
In October a new Centre for Intestinal Rehabilitation will be opened with high level medical/nursing support for patients with type 2 intestinal failure
Main duties of the job
There are opportunities for enhanced training, depending on the skills and educational requirements of individual applicants. This may include outpatient gastroenterology clinics, line access lists and endoscopy.
Candidates should hold full license to practice with the UK General Medical Council, have experience in nutrition and be a member of one of the UKs Royal Colleges of Physicians.

INFORMATION ABOUT THE DEPARTMENT
Gastroenterology in Leeds is based at St Jamess University Hospital. We also provide outpatient services at Leeds General Infirmary and Wharfedale General Hospital. We are a large yet cohesive department. We provide care for all aspects of gastrointestinal disease to a large local population and receive tertiary referrals into our subspecialty services.
We have 2 inpatient wards in the modern Bexley wing at SJUH. Both wards have excellent, well established teams of nursing staff and we are well supported by our experienced GI pharmacist, dietician and radiologists.
We operate up to 8 endoscopy rooms across Leeds, the majority of which have recently been refurbished with state-of-the-art equipment. We provide a wide range of diagnostic and therapeutic procedures, including bowel cancer screening, ERCP, interventional EUS, advanced EMR/ESD, stenting, capsule, double balloon and spiral enteroscopy and biliary manometry.
Our trainees gain valuable experience in managing complex patients within our subspecialty services. These include a large IBD service (supported by a team of IBD nurse specialists and weekly MDT with our radiologists and IBD surgeons), enteral and parenteral nutrition (including home PN), small bowel disease and pancreaticobiliary medicine.
There is a strong research background to the unit with our three academic consultants (Prof. Hull, Prof. Ford and Dr Subramanian). The research unit is based in the Leeds Institute of Molecular Medicine (LIMM) on SJUH site. There are existing research programmes in the molecular biology of colorectal cancer and IBD and current clinical research projects in IBD and endoscopy.

There are 20 consultant gastroenterologists, 1 specialty doctor with an interest in nutrition, 3 research fellows, 2 endoscopy fellows, 1 junior fellows, 6 NTN HST and 10 FY1/CT level doctors.
The consultants operate a consultant of the week on-call system and look after inpatients in 2 week blocks.

KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED
Candidates should hold a primary medical qualification and have a subsequent minimum of 4 years clinical experience, at least 2 of which should be at the level of core/internal medical trainee or higher. Candidates must hold a full Licence to Practice with the General Medical Council. Candidates should hold a certificate in Advanced Life Support from the UK Resuscitation Council, or an equivalent.
DUTIES OF THE POST
This post is embedded within the nutrition team, which consists of 4 specialist nurses; 4 specialist dieticians; and 5 pharmacists; with administrative support. The team is led by 2 consultant gastroenterologists, a consultant colorectal surgeon and a Nurse Specialist Manager. The team look after approximately 180 patients on home parenteral nutrition and see between 20 and 40 patients daily receiving parenteral nutrition in hospital.
These patients are very complex. Inpatients are reviewed regularly, with parenteral nutrition prescriptions altered to fit the clinical picture. Outpatients frequently also need close monitoring and a key part of the job is to help manage this.
The post-holder would be expected to work with the team, but also find educational opportunities within gastroenterology to match their needs. Other opportunities to develop skills in general gastroenterology through clinic and endoscopy work are encouraged. They would also be able to do more interventional procedures such as gaining more complex venous access, should this be suitable for them.
There is no on call associated with this job, which is based on a 40 hour week, Monday to Friday. Work in the morning principally involves seeing inpatients receiving parenteral nutrition. Afternoon work will either involve the weekly clinic/review meeting, or seeing drop-in outpatients with line/other issues.
The post is offered as a fix term 12 months contract. The post-holder will be allocated an educational supervisor who will assess progress and help mentor the successful candidate.

The post is fully supported, both by the non-medical roles within the nutrition team, but also the specialty doctor and medical/surgical consultants.
TEACHING AND RESEARCH/STUDY AND TRAINING
We have a weekly programme of departmental CPD activity. This includes histopathology and clinical governance meetings, journal article and clinical topic presentations. There are plenty of opportunities to develop teaching skills in ward based or more formal settings, including simulation.
