Job summary

An exciting opportunity has arisen to work as part of a dynamic locality team covering the EAST of Gateshead. The main focus of this role will be supporting the service users within the Sister Winifred Laver Promoting Independance Centre. The role will involves working closely with the local authority, St Albans GP surgery and the wider multidisciplinary team. The CNP will be expected to deliver high standards of clinical practice, ensuring patients receive safe and quality care.

Main duties of the job

To deliver of high standards of clinical practice that hold patients and their families at the center to ensure that they receive safe, quality care that gives them a good experience.

To identify and assess the core needs of patients and carers, being responsible for delivery of an identified package of care.

  • To assume 24 hour responsibility for a designated case-load.
  • To maintain standards of care through evaluating and implementing evidence based practice.
  • To lead and develop staff and the service provided.
  • To delegate work appropriately according to skill mix ensuring maintenance of safety within the community.
  • To support the locality Team Leader in achieving their objectives
  • To work effectively within a multi-disciplinary/agency team
