Job summary
This is an exciting opportunity to work as a Specialist Band 6 Physiotherapist using specialist clinical skills and clinical reasoning to provide a high quality Physiotherapy service within Frailty (Emergency Care / Virtual Wards).
The primary role is to work within the frailty pathway across acute and community but the role will also include management of other health conditions, particularly for patients presenting to emergency care.
The post holder will support suitable people referred to the service for planned early discharge from hospital or for the prevention of admission to the acute sector There is an expectation to support the service to develop through reflective practice, clinical supervision and audit.
There will be opportunities to allow you to develop a wide range of clinical, teaching and supervision skills. You will also work closely with the other team members and share responsibility for managing band 5, support staff and students.
This post includes aspects of community work, so post holder will be required to hold a full UK driving licence and have access to a car for work purposes.
You will be required to participate in 7 day working rotas; including shifts between the hours of 08:00 - 20:00 and regular weekends.

Main duties of the job

- To act as an experienced autonomous practitioner, providing specialised physiotherapy assessment and treatment programmed to patients within the speciality.
- To independently manage a caseload, using evidence based /patient centred principles to assess, plan, implement and evaluate interventions.
- To deliver care in both acute and community settings.

- To work as part of the multi-disciplinary team to deliver a holistic patient-centered approach to care.
- To regularly liaise with MDT members including those from heath, social services and voluntary agencies to ensure an integrated and comprehensive service is provided.
- To ensure that patients are identified for prompt discharge and handover to community services.

- To support the band 7 team lead and band 7 advanced practitioner physiotherapist in planning, coordinating, and delivering the service provided to patients and to assist in the development, planning and progression of service provision.

- To supervise physiotherapy students on clinical placements.
