Job summary

This is an exciting post as a Band 6 nurse with the community stroke rehab multi-professional team within Gateshead.

The post holder will work effectively within a multi-disciplinary/agency team attending to patients following a Stroke.

The post holder will provide specialist nursing with a defined caseload of stroke rehab patients, including assessment of care needs, secondary prevention and assisting in the implementation of rehab programmes.

This post is designed to support the Community Stroke Rehab team and the Team Lead fulfil their objectives, ensuring the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting.

Main duties of the job

As a Community Stroke Nurse you will be part of a community-based workforce providing specialised input as an independent and interdependent practitioner developing relationships across Gateshead.

You will assist individuals and their carers to proactively manage and anticipate their future health needs, support self-care, develop appropriate plans, and work collaboratively with the wider multidisciplinary team, JASRU QE Hospital and partner organisations such as, RVI hyperacute stroke unit, primary care, third party sectors and wider Integrated Locality Team members.

The post holder will support the team lead in delivering and evaluating high quality patient centered services in conjunction with the associated clinical teams to ensure this is incorporated within the Community Business Unit and Trust objectives, in line with Local and National Guidelines.

  • Be responsible and accountable for delivery of an identified planned or unplanned intervention working as an autonomous practitioner within their scope of competence
  • Provide high quality patient care in the community setting
  • To support the point of contact ensuring patients receive care at the right time, right place, by staff with the right skills.
  • Provide hands on care to patients in a variety of settings within the community
  • Maintain high standards of care through evaluating and implementing evidence-based practice.
  • Support the development of other staff and the service provided.
