Job summary
An exciting opportunity has come up for a one year post to coordinate the care of patients with liver cirrhosis and other liver diseases who are eligible for for HCC (Hepato-cellular carcinoma) surveillance. The successful person will work alongside the liver team to support and encourage patients to attend their appointments. They will also work to ensure accurate coding of patients in an outpatient setting.
Main duties of the job
Create a database of liver disease patients and track their eligibility for surveillance care, and ensure maintenance o this. Contact patients who do not attend scans and seek to rearrange their appointment. Follow up day to day enquiries from patients. Support data collection, and ensure accurate coding of outpatients.

Job responsibilities
Co-ordinate HCC surveillance in patients with cirrhosis
Ensure accurate coding of outpatients with liver disease/cirrhosis
Record and identify patients suitable for HCC surveillance, and report levels of surveillance back to central bodies.
Develop systems for ongoing identification and recording of patients with cirrhosis suitable for HCC surveillance in order to facilitate regular, automated reporting on HCC surveillance rates.
Follow up patients who do not attend HCC surveillance scans
Liaise with relevant departments in order to develop one-stop-clinics for patients with cirrhosis
Follow up patient enquiries on a day-to-day basis liaising with the hepatology nurses/consultants
Use good communication skills, basic clinical awareness and utilise appropriate tools and procedures, liaising with the nurses/consultant as appropriate.
Supports data collection for service delivery, audit, and quality surveillance.
In conjunction with nurse consultant will discuss and coordinate the care for a defined group of patients.
Organise and prioritise the designated workload in relation to identified needs.
Support the hepatology nurses in the collection of appropriate information to complete the holistic needs assessment documentation for patients with liver disease approaching end of life.
Communicate effectively with appropriate teams to ensure safe and effective transition of patient care between services and relevant information shared.
Support the hepatology nurses/consultants in delivering patient-centered, self-management support and education as necessary to non-complex patients, including how to self-assess (an understanding of what signs, symptoms, or situations to be aware of that would indicate concern).
Promote telephone advice line for patients/carers to use when they feel that their condition or needs have changed, including what to do out of hours
Act as point of contact for patients and carers and signpost all enquiries where appropriate to other departments in hospital or community setting.
