Job summary
An opportunity has arisen for an experienced nurse to join the Hospice at Home team as a Nurse Coordinator.
The post older will maintain the delivery of high standards of clinical practice that hold patients and their families at the centre to ensure that they receive safe, quality care that gives them a good experience. They shall work with Macmillan to ensure the team are delivering the six principles of End of Life care.
- To assume 24-hour responsibility for a designated case-load of complex patients referred to the service for domically end of life care.
- To maintain standards of care through evaluating and implementing evidence-based practice.
- To ensure the day to day coordination of the Hospice at Home team.
- To lead and develop a team of support workers and admin.
- To delegate work appropriately according to skill mix ensuring maintenance of safety within the community.
- To ensure the 6 principles of End of Life care are embedded.
- To work with the Specialist Palliative Care team/other Community teams to ensure optimum support to the patients at the end of life who wish to be cared for at home.
- To facilitate urgent response to patient need in order that the patient can remain at home or have rapid discharge from hospital.
- To lead changes in practice and evaluate the service model.
- To offer urgent review and assessment of patients in the acute trust who require fast track palliative discharge home.
Main duties of the job
Specialist Clinical Practice
- To assess the health-related needs of patients, clients, their families and other carers and identify and initiate appropriate steps for effective care for the individuals and groups. This may mean referrals to other services/community teams.
- To assess, plan, provide and evaluate specialist clinical nursing care to meet the care needs of individual patients and carers in their own homes.
- To oversee the provision of individual patient care by personal participation in order to maintain skills as an expert practitioner.
- To analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.
- To prescribe appropriate items from the Nurse Prescribing Formulary when required.
- To work with Trust policies and procedures that ensure patient safety at all times
