Job summary
The post holder will deliver nursing care to children and young people who have a Gateshead GP. Work is undertaken mainly in patient's homes, but some aspects of the service are delivered in a clinic environment or within schools and nurseries if appropriate.
Main duties of the job

The post holder will deliver clinical support to families, children and young people in their own homes or community. This includes safeguarding advice in conjunction with the Named Nurse for Safeguarding.

To lead and develop staff and the service provided.

To delegate work appropriately according to skill mix ensuring maintenance of safety within the community.

To support the Team Lead in achieving their objectives
- To assess and treat specific diseases in accordance with agreed medical protocols.

To assess, plan, provide and evaluate specialist clinical nursing care to meet care needs of individual patients and carers in their own homes.
- To analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.

To prescribe appropriate items from the Nurse Prescribing Formulary when required.
- To work as specialist practitioner within a multi-disciplinary of an integrated workforce that is based in the community.
