Job summary
The opportunity has arisen for a Band 7 specialist Palliative Care Nurse to join to integrated Specialist Palliative Care Team in Gateshead. This will involve a flexible approach to working across both hospital and community setting and 7 day service provision. This is a part time post 20 hour per week.
Key roles:
- To work effectively within a multi-disciplinary/agency locality based team managing complex patients identified as requiring advice and support during palliative and end of life care, ensuring that they receive appropriate interventions to be able to manage their symptoms and have their express wishes carried out with dignity and respect. These patients will have complex physical and mental health needs, and potentially numerous co-morbidities which may include frailty.
- To provide specialist advice and support to patients, carers and all professionals across Primary and Secondary care for palliative and end of life patients.
- To improve the patient journey by increasing access to assessment and appropriate care and treatment in line with best practice
- To provide an innovative patient centered service utilising advanced skills and knowledge in palliative and end of life care
- To apply standard principles of infection control to all activities at all times
Be a source of expert knowledge and expertise in palliative and end of life care to multiple stakeholders including patients, their families and carers
Main duties of the job
- Be responsible for the assessment, planning, implementation and evaluation of specialist programmes of care for the dedicated cohort of patients.
- Provide direct, skilled and individualised care for patients and their carers.
- Act as a specialist resource for patient and their family providing care and support that is responsive to their needs in particular the effective management of pain and symptom control
- To act in an advisory capacity for all professionals across the hospital and community by offering specialist knowledge, skill and advice regarding the care and treatment of those with palliative care needs.
- To ensure continuity and co-ordination of care between hospital and community environments and rotate into the hospital specialist palliative care team
- Make recommendations for service improvement and lead on the developments of new nursing initiatives.
- To participate in clinical supervision.
