Job summary
A fantastic opportunity has arisen to develop, lead and participate in specialist clinical pharmacy services within the heart failure teamat the QE in Gateshead.
We are looking for a motivated, enthusiastic and highly skilled clinical pharmacist, with a prescribing qualification and a keen interest in working in heart failure. The post holder will collaborate and run heart failure clinics, provide specialist clinical pharmacy input to the heart failure team and promote a seamless transition between primary and secondary care for heart failure patients.

Main duties of the job
The successful applicant will work closely as part of the MDT to review patient referrals into the service. They will lead on outpatient clinics and provide inpatient support, initiating and monitoring patients on guideline led therapies to optimise heart failure treatments.
As part of the team you will work closely alongside the the heart failure teams, cardiology and general practice teams to provide patient centered care.
You will share progress of role with the heart failure team, senior pharmacy team and the service line manager for medicine. There is also the opportunity for contributing to a regional and national work streams to share ideas and experiences.
A self-starter with an interest in clinical development, patient focused consultation and researchwill maintain our commitment to the trusts core values and join the team who have had work published and been nominated for national awards.
You will also support the inpatient clinical pharmacy team with clinical sessions on the ward or in the dispensary. You will participate in monthly group clinical supervision sessions and prescribing supervision sessions.

- To provide specialist clinical pharmacy input into the Heart Failure team
- To triage referrals for the team, ensuring relevant tests and information are available in advance of MDT discussions or clinics
- To work closely as part of the MDT alongside the heart failure team, cardiology and general practice to discuss referrals, generate treatment plans and escalate patients who require specialist input
- To carry out pharmacist led heart failure outpatient clinics which involves patient consultation, assessment, interpretation of test results, prescribing guideline-based medication and monitoring outcomes
