Job summary

The delivery of high standards of clinical practice that hold patients and their families at the center to ensure that they receive safe, quality care that gives them a good experience.

To identify and assess the core needs of patients and carers, being responsible for delivery of an identified package of care.

Based in the North East of England we provide a range of hospital and community health services from our leading facilities, including the Queen Elizabeth Hospital, Blaydon urgent care centre and Bensham Hospital, all within Gateshead.

Job responsibilities

  • To assume 24-hour responsibility for a designated case-load.

Specialist Clinical Practice

  • To assess the health and health related needs of patients, clients, their families and other carers and identify and initiate appropriate steps for effective care for the individuals and groups.
  • To assess and treat specific diseases in accordance with agreed medical protocols.
  • To assess, plan, provide and evaluate specialist clinical nursing care to meet care needs of individual patients and carers in their own homes.
  • To oversee the provision of individual patient care by personal participation in order to maintain skills as an expert practitioner and provide an effective role model for the nursing team and others in the multi-disciplinary team.
  • To analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.
  • To prescribe appropriate items from the Nurse Prescribing Formulary when required.
  • To work with Trust policies and procedures that ensure patient safety at all times
  • To work as specialist practitioner within a multi-disciplinary of an integrated workforce that is based in the community and is designed to meet the health needs of the identified locality.
    • Management and Leadership of a team of staff within community nursing services with accountability and responsibility for the safe delivery of all aspects of patient care within service portfolio.
    • Deliver hands on clinical care to a designated case load of patients within Gateshead as a caseload holder for a defined population of patients
    • Support the locality team leader in continuous service improvements and support the delivery of transformational projects within areas of accountability

    • Contribute to workforce development in line with team profile and national nursing and AHP picture.
    • Facilitate the development of clinical expertise and advances in nursing and AHP practice within the team in collaboration with the locality team lead, Clinical Operation manager and Clinical Lead.

    • To facilitate an environment where health promotion and informed patient choice is encouraged and developed to its maximum potential, within the available resources.
    • To evaluate clinical practice within the area by monitoring standards of care and ensuring that nursing practice is audited, and research based and supports the clinical governance framework.
    • To lead and facilitate an integrated approach to quality issues so that every member of the team contributes and is aware of their responsibility in this area.
    • To facilitate and ensure a safe, efficient and effective pathway for patients using the service.
    • To develop and participate in teaching packages for all grades of staff.
