Job summary

  • To work with as an integral part of the Diabetes in Pregnancy team supporting the patients journey in the clinic setting. Working closely with clinicians to manage, monitor and support the patient flow.
  • To support all members of the MDT to deliver high quality, safe patient care.
  • To ensure a safe working environment.
  • To assist the clinical staff to care for patients.
  • Adopt a flexible approach to provide support for Women's Health Clinic and Diabetes team
  • He/she will be responsible for the completion of a number of different administration tasks, supporting the diabetes midwives in delivering care to pregnant people with a diagnosis of gestational diabetes.
  • Collating patients' blood glucose readings for review by the Diabetes Midwives, contacting patients to assist with home blood glucose monitoring, completing regular audits and general administrative duties.
  • The post holder will be required to work closely with the multidisciplinary diabetes team and across a number of different IT systems. They must be comfortable with direct and frequent patient contact, communicating over the telephone and have the appropriate experience and skill set to support on an ad-hoc basis in the Women's Health Clinic as a Healthcare Assistant.

Main duties of the job

  • Collate weekly blood glucose readings for all pregnant people diagnosed with gestational diabetes for review by the Diabetes Midwives.
  • Support patients to complete and submit home blood glucose monitoring results in order to confirm/exclude gestational diabetes.
  • Register patients on blood glucose monitoring IT system following diagnosis.
  • Inform GP of gestational diabetes diagnosis and request repeat prescription for testing equipment.
  • Complete quarterly audit to ensure optimal referral and uptake of gestational diabetes screening.
  • Collate monthly gestational diabetes statistics.
  • Communicate with patients via telephone, SMS and email as directed/ supported by Diabetes Midwives.
  • Support team to follow-up any patient non-attendances.
  • Book/amend antenatal clinic and ultrasound scan appointments as directed by Diabetes Midwives.
  • Answer telephone calls, managing any enquiries that fall within clinical remit or appropriately escalating to a suitable healthcare professional.
  • Complete general administrative tasks as requested/directed by Diabetes Midwives or Clinic Manager.
