Job summary

The Community Forensic Outreach Service (FOS) has a vacancy for a Forensic Community Psychiatric Nurse and Care Co-ordinator. The core of the job is the management of a caseload of patients with forensic histories, acting as their care co-ordinator and as social supervisor as appropriate. You will alongside an MDT including Doctors, Psychologists, Recovery Workers, OTs and Social Workers.

This post holder will be engaged in effective risk assessment and management, as well as leading on engagement and interventions to support service user recovery and achievement of service user defined goals.

Specialist training will be provided for aspects of the post as necessary. Regular clinical and educational supervision will also be provided along with academic sessions as part of the weekly team programme. The team prides itself in providing a high-quality service to people with complex mental health problems across the 5 Boroughs that the Trust serves.

Main duties of the job

As a care co-ordinator, you will work collaboratively with other agencies, including police, MOJ, probation, housing, and residential providers (to name but a few), to ensure that service delivery is consistently of the highest standard, providing for safe and effective care and team support.

The successful candidate will be a qualified RMN or Learning Disability Nurse with sound experience in Mental Health, with knowledge and experience of forensic mental health service provision, offender care and be committed to team working.

The ideal candidate will also need to demonstrate good clinical knowledge and assessment skills, risk assessment and have sound knowledge of the Care Programme Approach. The post holder will be expected to be up to date with all mandatory training in relation to Trust policy and have current professional registration and knowledge as required by the NMC Code of Conduct.

Coordinating junior staff and the wider care team in providing hands-on care to service users and evaluating processes of care delivered. This will include: Undertaking 1-1 engagement with service users in an honest, open and non- judgmental way.

Recognising and valuing service users as individuals, acknowledging the importance of maintaining the service users respect and dignity at all times and ensure that the care provided respects equality and diversity.

Ensuring the monitoring and recording service user clinical observations are undertaken, such as physical observations and vital signs and treating or escalating as appropriate any signs or symptoms of deterioration or that does not fulfil the personal parameters to the relevant professional for treatment. Promote healthy lifestyles and give health promotion advice and support.
