Our Support and Development Programme will provide the following:

  • Work based support & supervision.
  • Caseload Support and Management
  • Training Opportunities
  • Additional coaching and supervision from our Nursing Develop Team.
  • Career Progression clinics and meetings with Head of Nursing
  • Preceptorship programme 10 sessions (If within first year of qualification)
  • Addition monthly sessions to support role development.

Main duties of the job

As a Community Mental Health Nursing Associate you will provide community patient led services and work with adults 18-65 years who have mental health issues, social needs and require support with their physical health needs by participating and leading within our health and well being clinics and visiting patients in the community to support their safety and recovery.

Our community multidisciplinary teams, comprise of nurses from Nursing Associates to Advanced Nurse Practitioners, occupational therapists, psychiatrists, psychologists, social workers, employment specialists, mental health practitioners, recovery support workers and peer support workers.

The teams are supported by good administrative systems and offers excellent working environments as we transform our older buildings into new with an ethos of team support and team ownership with supportive team managers and community leadership.

The Nursing Associate to Nursing will deliver high quality compassionate care. They will be competent to carry out tasks without direct supervision and will have an awareness of when to ask for guidance. They will be supervised by a Registered Nurse.

The Nursing Associate / Associate Practitioner will have completed a set of competency assessments that supports their practice.

Following assessments of patients, supporting the management of care needs through:

  1. Initial data collection
  2. Monitoring of individual patient progress
  3. Feedback and discussion
  4. Providing interventions and treatment
  5. Devising a plan of care and interventions supervised by a Registered Nurse.
  6. Supporting the fundamentals of care physical, mental health and social.
  7. To be aware of, and assist in meeting, the holistic needs of all patients, such as psychological, social, physical, recreational and spiritual needs
  1. Recognising and valuing patients as individuals, acknowledging the importance of maintaining the patients respect and dignity at all times and ensure that the care provided respects equality and diversity.
  1. Encouraging and empowering patients to have an optimum level of responsibility for their individual programme of care and with their consent, where appropriate, seek the cooperation of friends / relatives / carers, following recovery principles and approaches.
  1. Maintaining timely and accurate patient electronic records in line with Trust Policy, procedure and protocols. Use and complete care plans, pathway documents and Information System progress notes appropriately and in a timely manner.
  2. Subject to the requirements of a particular service area and within the confines of the role, following trust policy and upon successful completion of an approved programme of study and competency assessment signed by ward manager for the clinical area, the post-holder will administer routine medications.
  3. Undertaking, assisting and interpreting physical health care assessment and/or on-going physical health monitoring / review of patients, recording any changes in the service users physical health and reporting these to a Registered Nurse, acting appropriately on the advice given. Promote healthy lifestyles and give health promotion advice and support.
  1. Contributing towards the on-going quality of service and care and to identify any risk issues in relation to the patients health and social care. Informing the Registered Nurse / team if there are any: -
  2. Changes in the patients physical or mental state
  3. Significant factors relevant to the patient
  4. Incidents related to the patient
  5. Understanding and contributing to the Trust risk assessment policy, including being aware of the need to follow all safeguarding procedures; being aware of the necessity to safeguard and promote the welfare of children, young people and vulnerable adults at all times when working with patients.
  6. Where appropriate contributing to psychological interventions under the supervision of a registered professional.

  • Foundation Degree - or studying towards or equivalent level of educational attainment
  • Completion of Care Certificate and acquired competencies
  • English and Mathematics basic qualifications (e.g. GCSE at Grade C or above , New GCSE Grade 4-9 in Maths and English Language or evidence of Level 2 in Functional skills for numeracy and literacy).

Experience and Knowledge

  • Clinical experience minimum 1 year and experience of engaging with service users / patients
