Job summary

Sutton Home Treatment Team is seeking to recruit a permanent fulltime Nurse Practitioner band 6 RMN or a band 5 RMN who is keen to transit to a band 6 under the Community Nurses Development Programme with aim to attain a band 6; following successful completion of the Specific Competence Assessment at the end of 12 months of working for the service.

Sutton Home Treatment Team is a multidisciplinary, community based mental health team that aims to provide safe and effective home based assessment and treatment service; as an alternative to in-patient admission. The Service facilitates early discharge from the in-patient wards in the least disruptive and restrictive environment. It works in conjunction with, and is integrated with, other local service elements. The workload is team-based, and team members do not carry a specific caseload.

The Team is available to provide care and intervention 24 hours per day, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of Crisis intervention by the Home Treatment Team. The Service is available to Adults both Male and Female from 18 years old and upwards addressing an array of mental health difficulties and offer Crisis Intervention.

Main duties of the job

We provide home based high quality medical, nursing and OT interventions to service users in the London Borough of Sutton as an alternative to hospital admission.

You will;

  • work as apart of a team seeing service users in their homes in order to deliver nursing care.
  • have face to face and telephone meetings with service users, their families, and other professionals.
  • help develop care plans and recovery goals with service users.
  • administer medication to some service users.
  • assess risk and refer to other agencies as appropriate.

You will need to be able to drive and have a car.

Work Shift Pattern:

In this role you will be able to work 5 days and be off duty for 2 days in a week on a rotational rota basis to be able to meet the service demands. Hence, it is 37.5 hours per week, with a mixture of either early or late shifts at present. Early shifts commence at 08:30am and finish at 04:30pm; whereas late shifts commence at 14:30pm and finish at 10:30pm.

Patients and carers are supported by the Coral Assessment Team via the Mental Health Crisis Support Line 08000288000 if needed. Coral Assessment Team screens patients' referrals that might require HTT Crisis intervention during the day and at night.
