Job summary
Sutton Uplift has a fantastic opportunity for an experienced RMN or CPN to join the Adult Community Service piloting Transformation (improving access, recovery & service user experience). We are looking to fill a permanent full-time position (37.5 hours a week) over 5 days - typically Mon to Fri, 9am to 5pm (flexibility can be discussed at interview). This role will support the Primary Liaison and Recovery Team, which comprises of the Single Point of Access and the brief intervention Recovery Team.
You should enjoy working in a fast-paced but friendly environment where excellence in care is the paramount goal. You will be supported by a multi-disciplinary team (Admin, Consultant Psychiatrist, Senior & Trainee RMN/CPN's, Occupational Therapists and Managers), in order to provide the correct treatment and opportunities to support all patients.
Main duties of the job
Some of your responsibilities include, but are not limited to:
Providing therapeutic engagement for clients at primary care level and promoting independence as a step-down to GPs in primary care networks.
Participate in Duty rota e.g. resolve queries from GP's & other professionals, and support Team Lead in achieving key performance indicators by monitoring waiting lists (taking steps to mitigate breaches)
Screen incoming referrals, conduct assessments and identify/give options for best treatment pathway (seeking further professional advice & guidance where necessary)
Complete and maintain risk assessments and management plans
Occasionally support with home visits (conducted in pairs)
Keep clinical records up-to-date and in accordance with legislation and Trust policies
Receive and participate in clinical supervision in accordance with policy, and maintain compliance with Mandatory and Statutory Training
Provide support, guidance, and supervision to junior staff/students
