You will be expected to work closely with the services within the Acute and Urgent Care Service Line, the Community Service Line, the Local Authority and CCGs in a collaborative and professional manner. Main duties of the job

As the Team Leader you will be leading a community patient led service and work with adults 18-65 years who have mental health issues, social needs and require support with their physical health needs.

You will co-ordinate mental health referrals and lead with day to day problem solving and navigation, both within the service and at the interface with other trust services and external organisations. You will be working alongside the Clinical Manager and ACP to support staff well being, enhance and develop skills and knowledge through training and supervision to provide a team approach to excellent quality care.

You will be responsible for the monitoring and supervising of clinicians' caseloads using various IT systems, and you will work closely with the service's Senior Management Team to ensure the services delivered are of high quality and meet nationally agreed targets and local KPIs. We are currently going through our Community Transformation so during this exciting time you will have the opportunity to develop and enhance your team to meet the long term plan.

Our community multidisciplinary teams, comprise of nurses from Nursing Associates to Advanced Nurse Practitioners, occupational therapists, psychiatrists, psychologists, social workers, employment specialists, mental health practitioners, recovery support workers and peer support workers.

To manage and develop a responsive community mental health and social care service to meet the needs of the local population. To deputize for the clinical lead/manager in his/her absence
