Job summary

The Falls Prevention Coordinator aims to improve the health and wellbeing of patients at Homerton by identifying those at risk of falling, particularly older people or those who have fallen before admission. They will support ward staff in implementing appropriate treatment, equipment, or education to reduce falls. The role includes reviewing incidents involving harm or repeat fallers.

The post holder will work closely with A&E, inpatient wards, the Medical Day Unit, community services, primary and social care, leisure and voluntary sectors, and the community falls coordinator to ensure continuity of care across the falls pathway.

They will promote initiatives based on a sound understanding of inpatient falls issues, best practice, and current evidence. This supports the Trust's Self Care strategy by promoting independence and helping patients make informed decisions to reduce risk.

The coordinator will lead or contribute to the Trust's Inpatient Falls Committee, introducing and reviewing actions to minimise preventable falls.

This role suits a Registered Nurse, Physiotherapist, or Occupational Therapist with at least three years' experience working with older people who have fallen. The post holder is expected to maintain an active clinical role, undertake assessments, and lead by example.

Main duties of the job

  • To develop and implement an inpatient falls prevention policy & review its effectiveness.
  • To ensure adequate evaluation and assessment of older people who have fallen, to keep and provide accurate records and reports on the health status of the patient.
  • To work closely with clinical areas to set up systems to identify patients who have fallen.
  • To review in a timely manner the reported incidents regarding falls and target areas and departments who have the highest number of fallers to ensure patients are receiving appropriate follow up and review after a fall.
  • To ensure that targeted action plan/treatment of individuals at high risk of falling is being implemented.
  • To use Trust's risk management database and reporting procedures regarding the reporting of falls incidents and their follow up.
  • To take on an investigatory role for falls incidents including being involved in developing and implementing action plans.
  • To lead and actively contribute to the Inpatient Falls Committee.
  • Prepare written reports and present information about the inpatient falls work to a range of audiences.
  • To monitor service delivery through an effective audit system.
  • To establish a good working relationship with the primary care, social services and voluntary services.
  • Contribute to the promotion, maintenance and the monitoring of the City and Hackney Falls and Osteoporosis Integrated Care Pathway. To coordinate the implementation of the pathway across the sectors.
