SDEC Frailty Advanced Practitioner

Job summary

This is an exciting opportunity for a qualified Advanced Clinical Practitioner (ACP) passionate about the assessment and management of frail patients to work across Same Day Emergency Care (SDEC) service at Homerton Hospital.

The Homerton Hospital runs an established and successful SDEC unit for adult patients with undifferentiated acute medical conditions, with dedicated nursing, consultant and resident doctor cover. The unit benefits from a joint operational model with leadership from consultants in General Internal Medicine (GIM), Emergency Medicine and Elderly Care.

SDEC delivers acute hospital-level care without admission to an inpatient bed. This includes patients requiring ongoing treatment, investigation and monitoring that would otherwise have required an admission either by scheduling return visits to the service or management on the same day. Patients are referred into the service from:

  • General practitioners
  • Community and network partners (inc. NHS 111 and the London Ambulance Service)
  • Emergency Department
  • Acute medical take and medical specialties

There is also the opportunity to facilitate early discharges for a subset of patients from inpatient beds, where patients require ongoing treatment or monitoring and would otherwise have required a longer hospital length of stay.

Main duties of the job

The post holder will put the frail person at the centre of our decision making, ensuring we limit the risks of continued hospitalisation and excessive touchpoints with secondary care and support expedited discharges for those who have required a hospital admission. It is vital that they have a passion for the multidisciplinary assessment of older persons presenting with common frailty syndromes such as falls, confusion, immobility and incontinence.

The post holder is an experienced Advanced Clinical Practitioner with an accredited Master of Science (MSc) in Advanced Clinical Practice. The Advanced Practitioner should have an up-to date professional portfolio, mapped to the competencies and capabilities required of their role, setting, scope and specialism.

The Advanced Practitioner is service driven, and will be working within Health Education England's Multi-Professional Framework for Advanced Clinical Practice across the four domains of Advanced Practice as outlined below:

The post holder will work within the Frailty SDEC service Monday to Friday and support weekend working in Medical SDEC.
