Job summary

This is an excellent opportunity to lead on the management and ongoing development of a highly successful integrated multi-disciplinary service, offering Intermediate care and Reablement to residents of City and Hackney.

The Integrated Independence Team is well established and offers a range of interventions through its multiple integrated therapy-led pathways of Rapid Response and Home Treatment, as well as a mobile "morning falls" service and the care delivery services of Reablement (which is delivered on behalf of the local authority) and Rapid Care.

Working across all pathways an undertaking duties under the Care Act is our integrated social work sub-team, employed by London Borough of Hackney. Developments in 2024 have included the unique Paramedic-GP service "Paradoc" becoming an integrated pathway of the service, and the development of a new nurse-led Frailty Virtual Ward. Our efficient Single Point of Access for hospital discharge helps ensure we meet our KPIs month-on-month for response times and outcomes, and supports the Homerton achieve some of the most successful patient flow results in the country.

Main duties of the job

The Integrated Independence Team practitioners and clinicians maintain a strong and visible profile within the hospital, but especially within the busy Emergency Department, the Acute Care Unit and the older adults ward. Our MDTs are held weekly for both Home Treatment and Rapid Response and are all Geriatrician-led ensuring the best possible overview and planning for the patients we are proud to serve.

The postholder will:

  • Support the work of the whole service and provide supervision and guidance to the two team leads and senior social worker
  • Seek new opportunities for development and improvement of the service
  • Lead on the proactive engagement with commissioners and ensure the relationship with the local authority remains positive and productive
  • Ensure performance and quality reporting is undertaken with accuracy and to manage the complex funding and budgetary arrangement effectively
  • Advise on practice and complex clinical, social or practice issues in line with own professional registration
