Job summary
Do you love working with people with complex needs, supporting the transition from hospital through personalised care and support and facilitating independence and quality of life? An exciting opportunity has arisen for a band 7 occupational therapist to join the award winning Enhanced Practice in Care Homes (EPiC) Team in Hackney. The post is initially for 6 months but expected to extend until 31/3/2025, secondments will be welcomed.
Working as part of a small dedicated multi-disciplinary team, the EPiC Occupational Therapist works collaboratively within the 4 nursing homes in Hackney. Through the provision of proactive assessment for all new residents, and brief intervention focussing on posture and positioning, seating, falls prevention and facilitating independence and meaningful activity etc, the team has improved resident outcomes.
The postholder will be experienced working with older adults and adults with complex needs; will enjoy collaborative working and support the development, and education of care staff. The team uses QI methodology to review and improve the service and is keen to embrace innovation to maximise outcomes.
This fixed term contract (maternity cover) will provide the opportunity to lead on service development and evaluation with an aim to continue the service past April 2025.
Main duties of the job
The post holder will be an experienced Occupational therapist who will utilise specialist skills and knowledge gained from several years' experience to deliver person centred and clinically appropriate intervention to facilitate function, independence and improve quality of life for people living within a nursing home environment. They will work collaboratively with the care home staff, residents and families
This will be achieved by providing holistic assessments of residents and working with the patient and care home staff to enhance their knowledge, skills, confidence, and professionalism.
Responsibilities will include, carrying out initial interdisciplinary assessments for patients referred to the team, implementing occupational therapy assessment and intervention including the use and prescription of specialist equipment and liaising with and training care home staff to ensure recommendations are implemented through training and support. Using evidence-based/patient centred principles to assess, plan, implement and evaluate interventions, as well as responsibility for managing and transferring an individual patient's ongoing care to other professions and agencies as appropriate. Engaging with residents family and friends, and activity coordinators to explore and support meaningful activity within the care home environment.
