Job summary
The Adult Community Nursing service Single Point of Access is looking for a Senior Nurse Practitioner to lead its Clinical Assessment Team. This is an exciting opportunity for experienced community nurses to use their leadership and clinical skills to become a team leader. There will also be opportunities for the successful applicant to apply for Advanced Clinical Practitioner (ACP) courses.
Main duties of the job
The main duties are the line management and clinical leadership for the Clinical Assessment Team. The team assesses new patients referred to the service and delivers short term care in the patient's homes. Episodes of care include initial assessments, short term wound care and treatment, end of life care. The team is made of health care support workers, community staff nurses and nurse practitioners.
The successful applicant would be expected to complete rosters, manage sickness and absence, provide supervisions and complete appraisals and contribute to the development of the service. The applicants would also be expected to act as a clinical leader for the team, advising nursing staff on clinical management of patients, as well as undertaking home visits when necessary.
The community nursing service delivers care in patient's homes over City & Hackney. City & Hackney is an extremely diverse area. The Clinical Assessment Team endeavours to provide the best care for all our patients and ensure staff look after each other as well.
