Job summary

The Quality and Patient Safety Team is looking for two committed and innovative individuals with experience across all areas of quality and safety to join our team as Patient Safety Incident Investigators. These are brand new roles, and with a new Patient Safety Investigation Lead will make up our new Patient Safety Investigation Team, a key part of our implementation of the new Patient Safety Incident Response Framework.

This is an exciting time to be joining an enthusiastic and collaborative Quality and Patient Safety team, who are working to deliver safe andeffective care to our patients. We believe PSIRF offers a real opportunity to improve the way we investigate and learn from incidents, and thispost is a key role in making this a reality.

Main duties of the job

The Patient Safety Incident Investigator will take the lead in investigating and reviewing incidents on behalf of the Trust, and will support patients, families, and staff through the process of an investigation. The post holder will conduct thorough investigations into patient safety incidents and other incidents, as necessary, using dedicated learning responses and ensuring Being Open principles are applied. When engaging with those affected, the purpose is to provide explanation and support through what can be a difficult process. This role will include dealing with and providing support to newly bereaved individuals some of whom will be deeply distressed.

Essential

  • Educated to Masters level or equivalent experience.
  • Completed the PSIRF - Systems approach to learning from patient safety Incidents 2 day training course (or has planned to)

Desirable

  • Qualification in a risk / governance / patient safety related discipline at diploma or equivalent

Skills / abilities

Essential

  • Knowledge and experience of risk management and clinical governance processes essential, with an ability to challenge and question clinical practice of all members of the multiprofessional team
  • Ability to analyse and rigorously review all aspects of the patient's care pathway, interpreting information and using knowledge and judgement to provide highly specialist advice to recommend modifications where necessary
  • Compassionate with the ability to deal with distressed and highly emotive patients/relatives

Desirable

  • Ability to use electronic information databases EPR, RiO etc

Experience / knowledge

Essential

  • Expertise in completion of systems based investigations
  • Understanding of the Patient Safety Incident Response Framework and the National Patient Safety Strategy.
